Description:
This position involves working in a case management role on a day shift.
The role requires case management for adult and geriatric patients.
The candidate must have experience in care coordination and discharge planning.
Disease management experience is also required.
The position includes working every fourth weekend.
The candidate must work 2 out of the 3 winter holidays.
While previous charge experience is not mandatory, it is preferred.
Experience with a charting system, particularly Epic, is necessary.
Candidates must be willing to float within their scope of practice as needed.
There are pre-approved time off policies in place.
The educational requirement is a Bachelor of Science in Nursing, preferred.
Experience with needs assessment and order of durable medical equipment is a plus.
The ability to create a plan of care is essential.
This position requires direct support and coordination for patient care.
