Job description

Shift: Primarily day shift, but must be flexible to meet departmental needs

Status: Full-Time, 10 days per biweekly pay period

Weekends: No

Holidays: No

We currently have an opportunity for a Population Health RN to help maintain population health services for chronic care management and transitional care management patients. This is a remote position. This position reports to the Clinical Quality Manager.

Experience/Qualifications:

RN Graduate of accredited School of Nursing/College of Nursing

CPR/BLS certification, required within 90 days of hire

Current, unrestricted RN license

Care Management and Transitional Care experience is preferred

Physician Office experience a plus

Prior experience with Microsoft Office and EPIC is preferred

Responsibilities:

Demonstrates ability to exercise effective clinical reasoning related to the assessment, care, delivery and evaluation of patients of all ages.

Functions as a team player to focus on patient management as a provider support.

Collaborates with appropriate staff to determine level of care needs beyond acute care.

Assesses the need for preventive, restorative and health promoting activities for patients.

Communicates effectively with patients, families, community resources and many other medical professionals.

Serves as a liaison between clients and office providers.

Effectively educates patients.

Establishes a good working rapport with the office staff and all internal/external customers in order to achieve maximum productivity and efficiency.

Responsible for providing Annual Wellness visits.

Demonstrates effective problem solving skills in role.

Perform other projects, tasks, or duties as assigned and/or as needed

Skill/Ability:

Ability to work independently and as a team player, to exercise good judgment and successfully manage a variety of tasks.

Ability to think critically including interpretation, analysis, and evaluation.

Ability to demonstrate competencies with applicable policies and procedures as outlined in the Orientation Program prior to the completion of orientation.

Individual is self-motivated with strong organizational skills and a positive attitude.
