License Hospice Social Worker (PRN)

PRN License Medical Social Worker (LMSW) needed for a new hospice company in San Antonio, TX.

Our Social Workers are responsible for the delivery of various social work services to hospice patients and their families. Our Social Workers provide social service counseling and guidance to facilitate the best personalized care for each patient.

Responsibilities:

• Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the RN Case Manager and other members of staff including the interdisciplinary group.
• Assess the relationship of the patient's medical and nursing requirements to the patient's home situation, financial resources, and availability of community resources, to include funeral arrangements if needed.
• Supervises the implementation of an individualized social work plan of care and notified Bereavement Coordinator of bereavement assessment needs.
• Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses and symptoms of the terminal illness.
• Counseling patient and family/caregivers as needed regarding to stress and other identified coping difficulties.

Qualifications:

• Masters of Social Work from an accredited school of social work
• Minimum one (1) year experience as a social worked in long term care or medical surgical/acute care setting.
• One (1) year of hospice experience preferred but not required.
• Must be able to work flexible hours and travel between offices, facilities, etc.
• Must be computer proficient in typing and various programs, including background in EMR.
• Must have ability to handle stress associated with ministering to patients/residents and their loved ones who experience pain, suffering, death and grief.
• Possess and maintains current CPR certification.
• CMS requires that all patient-facing staff, as well as those that are in contact with patient-facing staff, be fully vaccinated, or have a documented religious or medical exemption. This important rule is required for all healthcare organizations that receive federal funds through Medicare or Medicaid.
