Description

Crown Health is a fast-growing healthcare practice with a unique approach to geriatric care. Based in Kent, WA, our Mission at Crown Health is to provide safe, compassionate, reliable primary care to vulnerable patients in the comport of their home. Our amazing staff of Physicians, Nurse Practitioners, Physician Assistants, and Medical Assistants provide care through both in-person and tele-health visits.


The Crown Health Primary Care Medical Assistant delivers in-home and virtual clinical care support through hands-on collaboration with our clinicians at the bedside and direct engagement with our patients and their caregivers to ensure our patients get the right care, in the right place, by the right person, and at the right time. Key activities include performance of health screenings and bedside diagnostic and treatment procedures under supervision of clinical practitioner, clinical documentation and order entry scribe support, virtual care facilitation, clinical visit preparation patient education and routine care coordination support. This position reports to the Practice Manager.


This is an ideal position for a medical assistant who seeks an opportunity for hands-on experience with patient care delivery at the bedside, enjoys a collaborative multidisciplinary team-based approach to care, and is excited to engage in developing and nurturing our innovative, value- based clinical model focused on caring for patients with complex needs who are inadequately served by traditional healthcare delivery systems.


This position offers professional development support with in-depth training opportunities and real-time support from our on-staff experts in home-based care delivery, clinical operations and practice management, complex care management, geriatric medicine and palliative care, clinical pharmacy, and social determinants of health and health equity.


RESPONSIBILITIES

  • Medical assistant support at the bedside for in-home primary care visits including measuring vital signs, administering health screenings, documenting medication reconciliation, and maintaining up-to-date electronic health records for vaccination history, preventative health screenings, and other quality measures.
  • Clinical documentation and order entry support for in-home patient encounters under the direction of the rendering clinical provider (i.e. bedside scribe function).
  • Laboratory specimen collection and preparation (includes blood, urine, stool, saliva, sputum, nasal and oropharyngeal swab, and superficial cultures) and performance of diagnostic point- of-care testing supported by CLIA waiver under the supervision of a clinical practitioner.
  • Performance of select clinical procedures including venipuncture, EKG, ABI, spirometry, basic hearing and vision tests, removal of sutures/staples, preventative and maintenance foot care, and medication and vaccination administration under supervision of a clinical practitioner (with scope of work and level of supervision as determined by state regulations).
  • Facilitation of virtual visits at the patient bedside to support care delivery under the direction a remote clinical practitioner.
  • Visit preparation and after-visit care coordination as needed including tracking results for diagnostic orders and referrals, managing quality measure orders and results, coordinating routine care scheduling needs, and ensuring appropriate follow-up through collaboration with practice management team.
  • Delivery of exceptional customer service to our patients, caregivers, and clinical partners including proactive, timely, and comprehensive communication from a holistic patient- centered perspective.
  • Accurate and timely documentation in the medical record of all patient care support activities.
  • Promotion of and participation in patient engagement and experience initiatives and efforts.
  • Supportive function as growth ambassador and community outreach liaison to foster
  • relationships with patients, caregivers, congregate living facility staff, and partnering
  • organizations.
  • Active participation in care team huddles, operational huddles, organizational meetings,
  • and clinical partnership meetings as requested.
  • Adherence to compliance policies, procedures, and standards of conduct including all
  • applicable laws and regulations.
  • Other market-specific duties as assigned.

QUALIFICATIONS


  • Completion of formal medical assistant training program and/or commensurate clinical experience.
  • Current licensure in applicable state(s) or ability to obtain licensure in states that require medical assistant licensure.
  • Medical assistant certification preferred.
  • Basic life support (BLS) certification required.
  • Prior experience caring for aging adults and/or adults with complex care needs preferred.
  • Prior experience caring for patients in the home setting including private homes and/or
  • congregate living facilities preferred.
  • Average to advanced computer and software skills required.
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; valid driver license and proof of liability and property damage insurance on vehicle used is required.
