Job Description

ESSENTIAL FUNCTIONS, DUTIES AND RESPONSIBILITIES
The essential duties and responsibilities described below are representative of those an employee encounters while performing the basic functions of the position. 1. Communicate with community healthcare professionals, including physicians, discharge planners, nurses and state agencies on the care needs of all residents at the community.
2. Perform nursing duties as directed and in accordance with the appropriate scope of the Nurse Practice Act, including administering medications and performing treatments as ordered by physician.
3. Conduct thorough reporting of pertinent information regarding resident care to oncoming shift. Complete documentation in and encourage use of the Communication Log on each shift.
4. Participate in end-of-shift report to ensure all needed information is communicated.
5. Coordinate incidental medical and dental care for residents.
6. Assist Executive Director or Journey Program Director with physician orders and pharmacy delivery of medications. Organize central storage and delivery of medications for each shift.
7. Observe residents and collect data pertinent to resident care. Communicate all relevant resident information with physicians and other healthcare professionals as needed.

8. Ensure proper documentation regarding change in condition and incident reporting for both residents and staff. Ensure all paperwork is completed and that proper documentation is maintained in the medical record.
9. Assist Care Managers and Medication Aides in the performance of their job requirements and assume caregiver and/or medication aide activities when needed.
10. Respond promptly to resident and staff needs. Carry a pager and radio at all times while at the property.
11. Assist both Care Managers and residents in performing activities of daily living, including; shaving, dressing, oral hygiene, toileting, bathing, incontinence care, feeding, nail care, transferring in and out of beds/wheelchairs, and the setting and clearing of tables at meal time.
12. Use Universal Precautions and follow established policies concerning exposure to blood/body fluids.
13. Work towards maintaining each resident’s self-respect, personal dignity, personal safety and confidentiality.
14. Display tact and friendliness when dealing with residents, families, visitors and co-workers.
15. Assist new residents with arranging personal items and help familiarize residents and their family members with their new surroundings.
16. Encourage residents to participate in daily activities such as bed making, laundry, and setting tables.
17. Assist Activity Department staff in carrying out planned activities.
18. Attend all scheduled in-service classes.
19. As requested, assist Executive Director or Journey Program Director with orienting new staff to the residence.
20. Provide support to community’s marketing efforts through positive/friendly interaction during all property tours and telephone contact with the public, and as directed by a supervisor.
21. Maintain a safe and secure environment for all staff, residents and guests, following established safety standards. Perform daily and nightly rounds of the property. Notify maintenance staff of all needed repairs.
22. Encourage teamwork through cooperative interactions with co-workers and other departments.
23. Support a positive and professional image through actions and dress.
24. Performs other duties consistent with the position as assigned by the Executive Director or Journey Program Director.

MINIMUM QUALIFICATIONS
Education: Must 18 years of age or older.
Licensed Vocational or Registered Health Services Director in the applicable state.
Experience: Prefer one (1) year experience in a health care environment, working with cognitively impaired older adults.

Mathematical Skills: Ability to perform mathematical calculations consistent with medication delivery.

Reasoning Skills: Ability to work patiently with cognitively impaired individuals. Ability to analyze stressful situations and conceive creative courses of action. Oral/Written Communication Skills Ability to read, write, and speak English. Ability to explain work duties to staff. Ability to follow oral and written directions. Ability to communicate effectively with physicians and other health care professionals. Ability to communicate effectively with residents and their families.
Equipment Used: Standard household equipment (dishwasher, iron, vacuum cleaner, washer, dryer). Basic office equipment including computer.

Physical Effort: Be mobile and able to perform physical requirements of the job. Ability to assist cognitively impaired people with activities of daily living. Good physical health verified by a health screening, including a chest x-ray or an intradermal test result not more than 6 months prior or seven (7) days after employment.

Personal Characteristics: Displays sincere compassion toward cognitively impaired older adults. Demonstrates genuine concern for the physical and emotional needs of older people and their families.

Working Conditions: Air-conditioned, well-lighted health care environment. Other: (As required by Title 22) Employee must pass a criminal record clearance prior to beginning work in the community. Employee must possess current first aid training.
