SALARY RANGE: $22.00 - $25.00/Hour

JOB SUMMARY:
The Health Education Specialist uses health education, goal setting, motivational interviewing, and other proven techniques to drive healthy change. As an integrated member of the clinic/care team, the Health Education Specialist provides professional-level skills in teaching, and helping patients achieve their maximum health and wellness goals through educational programs and guidance. The Health Education Specialist will also assist patients to access and navigate HOPE clinic services. The Health Education Specialist may also contribute to the development of new health education programming based on patient, clinic, and community needs.
MAJOR DUTIES & RESPONSIBILITIES:
- Implement health education programming in one-on-one, co-visit, and group settings to increase the knowledge and self-efficacy of patients to prevent illness, manage health conditions, and improve overall wellness. Core topics will be determined based on clinic assignments and may change.
- Assist patients in setting goals related to their health based on assessments of patient needs (behavioral, social, and medical), readiness for health behavior change, and desired outcomes.
- Identify and/or develop and then maintain presentations, handouts, equipment, models, and other teaching tools and instructional materials to convey information to patients and community members effectively.
- Research health education issues;
- Participate in the design and testing/evaluation of new health education curricula and teaching materials to support patient, clinic, and community needs.
- Develops curriculum and train staff about specific health topics that may be presented by others in one of the other languages spoken at HOPE Clinic.
- Securely and accurately document all patient information and encounters into EMR;
- Advanced MS Office skills or other media.
- Meet assigned performance metrics including those related to grant/contract requirements, if applicable.
- Ability to anticipate challenges and effectively resolve potential issues through creative problem-solving.
- Ability to work creatively and with flexibility in a fast-paced environment while maintaining high work standards.
- Ability to develop rapport and engender trust.

- Serve as a member of an interdisciplinary clinic, and care team, and promote effective working relationships with internal and external stakeholders.
- Ensure that work assignments and information gathered from day-to-day work is not shared with anyone and protect all patient and agency information.
- Knowledgeable of and maintains HIPAA standards of privacy and confidentiality;
- Ensure that all patients receive the best possible care;
- Attend on-site/off-site community engagement and/or agency activities, meetings, trainings, and workshops events as needed; including travel to different sites
- Perform other duties as assigned to support HOPE Clinic’s Mission, Vision, and Values.
QUALIFICATION REQUIREMENTS:
- Bachelors Degree in health education with certification preferred;
- 1+ years experience in healthcare
- Experience with curriculum development.
- Excellent written and verbal communication skills.
- Good interpersonal skills.
TRAVEL REQUIREMENTS:
- Ability to travel to satellite sites and/or off-site locations for support/assignments, events or training as needed;
- Proof of liability and property damage insurance on vehicle used is required.
EDUCATION and/or EXPERIENCE:
Bachelors Degree in public health, social work, healthcare administration, or related field; masters advantageous
OTHER SKILLS and ABILITIES:
- Bilingual in another language served by HOPE Clinic as well as English is preferred.
- Above average skills in language ability as well as public speaking and writing.
- Must have good transportation and a valid Texas Driver’s license.
