Social Worker

The Carmelite Sisters came to the Palm Beaches in 1960. Later named the Lourdes-Noreen McKeen Residence, the facility was the first of its kind in the area to provide nursing, long-term and seasonal housing for the elderly, all with the signature type of care that distinguished the order's approach from others. Today, our senior living community offers skilled nursing, short-term rehab, independent living and assisted living apartments.

We are looking for a Social Worker.

• Full-time schedule plus weekends required.

Social Worker Qualifications:

• Bachelors or Masters in Social Work required.
• 1 year of Long-Term Care experience required.

Social Worker Job Summary:

• Assist in the implementation of the social service programs of this facility.
• Participate in discharge planning.
• Interview residents/families as necessary.
• Refer the resident/family to the appropriate social service agencies.
• Provide information to the resident/families as to medicare/medicaid, and other financial assistance programs.
• Coordinate social service activities with other departments.
• Maintain accurate and timely service progress notes.
• Serve on committees as appointed by the Administrator.
• Assure the development of preliminary and comprehensive assessments of the social service needs of each resident.
• Assure the development of a written plan of care (preliminary and comprehensive).
• Welcome new residents on the day of admission.
• Run errands for residents as required.
• Interpret social, psychological, and emotional needs of the resident/family to the resident care team members.
• Visit residents on a daily basis allowing them to express concerns/problems.
• Discusses residents/family problems with the Director on a daily basis.
