Starting Pay Rate: $30.00
Full Time or Part Time: Full Time
Standard Hours Per Week: Monday - Friday, 40 hours/week
Work Schedule: Shift starts between 6:30 AM and 8:30 AM MT
Schedule Type: Hybrid, Remote

Responsibilities:

As a Care Coordinator, you will conduct reviews of care plans, assessments, and other various medical records against policy language to determine eligibility for Long Term Care (LTC) claims. Once eligibility is determined, you will develop individualized, cost-effective care plans as a guide for caregivers and as a tool for SILAC's care management. You will also be responsible for signing Chronically Ill Certifications for all tax-qualified policies on claim at least annually.

This role requires thorough research skills, attention to detail, handling of sensitive health and personal information, as well as the ability to communicate complex information in an easy-to-understand manner.

Some administrative duties are intertwined in the role of a Care Coordinator. These duties may include research and resolutions of situations that vary in nature. SILAC provides ongoing training to enhance the knowledge and accuracy in responding to inquiries of diverse complexity.

SILAC is looking for a detail-oriented individual that possess the technical know-how to review Long Term Care claim requests to determine eligibility and manage cost-effective care planning for our policy owners.

Requirements:

  • 1-year experience in care management or 2 years clinical experience (preferably with geriatric clients) assessing, planning, and coordinating care.
  • Bachelor of Science in Nursing, and licensed as a Registered Nurse (RN), Licensed Practical Nurse (LPN), or Licensed Clinical Social Worker (LCSW), or other similar licensure or degree.
  • Ability to accurately assess policy owner needs and create individualized care plans.
  • Experience in communicating professionally, patiently, and clearly in speech and in writing.
  • Ability to research and logically consider details from many sources, and to bring them together to support a conclusion.
  • Demonstrate an understanding of the relationship between insurance claims paid by SILAC and rates charged.
  • Experience using PC applications such as MS Word, Excel, Teams, and Outlook.
  • Detail-oriented with the ability to complete tasks correctly and in a timely manner.
  • Ability to work independently and effectively deal with stress caused by workload and time deadlines.
