$15,000 Sign-On Bonus

Responsibilities

Provide direct medical care to PACE participants
Complete sections of assessments and care planning
Manage a panel of participants
Initiate, evaluate, review and change orders for medications and diagnostic studies or treatments with approval
Assess, initiate and coordinate care on a daily basis under supervision
Document all participant interactions, assessments, next steps in the EMR within the specified timeframe set by the center
Make referrals to specialists with approval and request consults as needed
Perform history and physicals and participant teaching
Order, interpret and follow up on lab tests and results
Perform selected procedures
Recognize and escalate abnormalities as needed
Liaise with other team members for participant's care, including behavioral health and pharmacy
Evaluate and treat participants where they are located, whether that be in the center, home, virtually, SNF etc
Facilitate advanced planning discussions with participants
Document chronic conditions via HCC capture
Provide on-call coverage as needed

Requirements

Unrestricted California Nurse Practitioner License with Prescriptive Authority
Master's degree and graduate of accredited NP program
CPR Certification
Fluent in Spanish
Passion and mission orientation for serving high-risk seniors and frail older adults Experience with complex and older populations
Strong communication and interpersonal skills Ability to lead independently while also working collaboratively with others in a team setting
Experience as an RN for 2 years with complex populations, prior to becoming an NP
Current knowledge of chronic care/geriatric issues and best practices
Proof of a valid CA driver's license, personal transportation, good driving record and auto insurance as required by State law
