Job Description

Assists the Director of Access and Provider Partnerships in the clinical management of the Admissions Department. Serves as a professional clinical leader of the Admissions Department, including serving as a liaison between the Admissions Department and other areas of the organization. Responsible for assigned functions related to providing information about services to potential hospice patients and coordinating the admission process.

This position serves as the professional and administrative leader directing, planning and assuring the effective management of the Admissions Department. Responsible for implementing strategic plans to increase referrals and admissions of appropriate patients. Responsible for oversight of the referral and admissions processes that ensure that care and excellent service are made available to our healthcare partners, patients and families.

Qualifications

  • Bachelor’s Degree preferred.
  • Experience should include case management or admission experience in an acute care or long-term care setting. Registered nurse (RN) required with a minimum of three years’ experience, including one year in an acute care setting preferred. Hospice experience/leadership experience preferred but not required.
  • Knowledge of Hospice Conditions of Participation (COPs) Preferred.
  • Ability to execute an effective, supportive, sensitive approach in response to the needs of patients, families, and staff.
  • Must be Certified Hospice and Palliative Registered Nurse (CHPN) within 12 months (if applicable).
  • Effective oral and written communication skills. Ability to develop and apply effective customer service skills. Demonstrates organizational, administrative, and people management skills.
  • Flexible, creative, assertive, articulate, nurturing and self-nurturing.
  • Ability to multi-task, work well under stress and with a sense of urgency to meet deadlines.

Key Result Areas

  • Responsible for overseeing the successful operation of the admissions department, including forecasting and fulfilling referral, admissions and census goals for the organization based on market data.
  • Trains, mentors, and coaches staff to ensure excellent customer service, high level staff performance and clinical excellence as measured by positive feedback from the Family Satisfaction Survey, QI team performance, and integrity of staff documentation.
  • Teach and uphold Treasure Coast Hospice’s standards with a focus on perfect documentation, perfect visits, perfect communication, and professional dress & behavior.
  • Lead, develop and motivate employees with confidence, energy, integrity and resources.
  • Oversees the Admissions team under the guidance of the Director of Access and Provider Partnerships, to ensure that the referral/admission process is performed in a smooth and coordinated manner. Serves as a resource to the entire Admission team, including Intake staff, and works closely with the dedicated Admission physician.
  • Oversees the Admissions RN Preceptor Program while working closely with our Senior Preceptor.
  • Evaluates the effectiveness of the referral process through interviews, periodic meetings and provider satisfaction surveys of health care and community partners. Tracks and manages ongoing departmental quality through review of key performance indicators.
  • Ensure adequate staffing levels; creates and monitors monthly employee schedules; assists in ensuring proper orientation of new employees.
  • Monitors departmental productivity and ensures that the productivity of the Admission nurses is consistent with the standards and goals of the department’s conversion rate. This includes timely follow ups of pending patients to ensure admission to hospice program.
  • Assists with facilitating and attends staff meetings on a regular basis. Answers and/or channels suggestions, criticism and questions to appropriate persons.
  • Communicate on a continual basis with the Director of Access and Provider Partnerships regarding areas of concern and/or difficult issues related to both patients and employees to assure that needs of both groups are met.
  • Screen and process inquiries for admission to hospice care.
  • Assists as needed to determine patient’s appropriateness for hospice services consistent with hospice service policies and admission criteria in conjunction with patient’s physician and medical director.
  • In collaboration with the Director of Access and Provider Partnerships, responsible for conducting performance evaluations effectively and efficiently.
  • Maintains a professional demeanor, to include professional dress, dependability, and effective and timely communication with employees, leaderships and outside contacts. Works diligently for positive outcomes.
  • Must provide initially and update as appropriate all required information for credentialing at area healthcare facilities and hospitals, and maintain good standing with each.
  • Participate as an active supporter of the Treasure Coast Hospice compliance program, including the emphasis to report, as well as complying with all Federal, State and Local laws as well as the organization’s compliance program.
  • Fulfill all responsibilities related to the success of the strategic plan of the organization, as requested and assigned.

OSHA Category

Position will be categorized as category II (involves no routine exposure to blood, body fluids/tissues but may occasionally require performing unplanned category I tasks).

Working Conditions

  • Typing or otherwise working primarily with fingers rather than with the whole hand as in handling.
  • The work is required to have close visual acuity to perform an activity such as/preparing and analyzing data and figures; transcribing, viewing a computer terminal; extensive reading.
  • Lift up to 50 pounds frequently.

Additional Information

Incumbent may be required to perform emergency duty before, during and/or beyond normal work hours or days in the event of an emergency, crisis situation or disaster (man-made or natural) including evacuation sites. The person in the position will work in a smoke-free location, and is expected to adhere to all smoking restrictions.
