Job Description

Caringplace Adult Day Services that provides robust care and innovative programs serving the frail elderly and dementia-impaired adults and their caregivers is looking for a Nurse/Supervisor.

SCOPE OF POSITION: The nurse/supervisor of care assistants is responsible for supervising the care assistants, their schedules, and overseeing the direct care provided to the Caringplace clients.

SUPERVISION AND RESPONSIBILITY: The nurse/care assistant supervisor is supervised by the Executive Director and is subject to all policies, guidelines, and procedures of Caringplace.

SPECIFIC RESPONSIBILITIES: 1. Interview, hire, supervise, evaluate and terminate care assistants. 2. Provide orientation and training to new care assistants regarding their tasks and responsibilities. 3. Create, monitor, and implement the care assistants' schedules to include shifts, breaks, lunch time, vacation and paid time off. 4. Supervise and administer the care assistants' leave requests and records. 5. Monitor and provide teaching and counseling regarding the direct care and staff/client interactions provided by the care assistants. 6. Schedule and direct weekly team meetings. 7. Develop and implement regular staff in-services related to Montessori principles in collaboration with the Program Leader. 8. Document as needed medical information, progress notes, incident reports and other information concerning the health and well-being of the clients. 9. Provide assistance to clients in medication administration that is safe and accurateDocument said assistance. 10. Organize and maintain all client medications in a safe and secure environment. 11. Participate in family meetings of new client assessments. 12. Participate in activity programs to monitor the care of clients and to help Program and activity leaders remain aware of any physical limits of clients. 13. Maintain accurate and timely health information in the clients' records to include: weekly medication reports, progress notes, incident reports, and daily staff records. 14. Maintain adequate first aid supplies and emergency medical supplies as outlined in the Center's disaster plan. 15. Monitor infection control and excellent health care practices within the Center. 16. Provide information, referral, and consultation to families regarding the health and well-being of the clients. 17. Consult with external medical professionals and care providers as requested by families and within the scope of the Center's confidentiality policy. 18. Perform other duties as assigned.

POSITION REQUIREMENTS: 1. Registered Nursing Degree, Bachelor’s 2. Must provide documentation of negative TB skin test and update annually. 3. Must show proof of Covid immunizations update with boosters or as recommended by the CDC when working with vulnerable, at-risk populations such as will be the case at Caringplace; criminal background report at Caringplace expense. Must sign permit for random post-hiring drug testing as implemented by Caringplace policy. 4. Must provide documentable references and work history experience with elderly, especially cognitively impaired; and able to withstand the demands necessary to perform the job. For example, you may need to intermittently lift and move objects up to 25 pounds. Must be able to assist clients with standing and transferring from a chair or wheelchair if needed. While there are no bed clients at Caringplace, there could possibly be wheelchair clients who are capable of standing and pivoting with assistance.

OTHER CONSIDERATIONS: 1. Must sign a Staff Confidentiality Agreement compliant with HIPAA regulations before beginning work at Caringplace 2. Must maintain CONFIDENTIALITY of: client names, conditions, behaviors, and presence at Caringplace, current or past; caregiver identification and relevant information; and Caringplace operations including information about co-workers, business protocols and processes, plans, and shared information, unless management has made it public through the media. 3. CONFIDENTIALITY: means that information you know or observe while working at Caringplace is not to be shared with your family, friends, current or past coworkers, or others; this includes, but is not limited to, client names, healthcare, medication, and behavior. Any information you need to discuss or have questions regarding may only be discussed with your supervisor. 4. Employer Provided Training: Must attend and complete a two-day workshop at Caringplace’s adult day center, prior to working with our clients, at the Caringplace’s expense, to become certified in basic principles and techniques of Montessori-based dementia programming and strive to implement these techniques within your work and direct assistance with clients. This is an investment Caringplace agrees to invest in all its staff upon hiring and prior to working independently with day-stay clients. Continuing education in these and other skill enhancements will also be expected as directed and provided by employer. The Center, and its campus is a designated smoke-free area, and due to our commitment to and liability for our clients who are entrusted to our constant oversight and care during their contracted day(s) at Caringplace, leaving the premises of Caringplace is not permitted during your scheduled work shift, unless an emergency or a previously scheduled two-hour absence is required to take care of personal business. Caringplace provides a free lunch and snacks as a benefit with wages paid through lunchtime as all staff are responsible for monitoring and sitting at a table with up to three clients.
