Position Summary: As a pharmacist at Ironwood Cancer & Research Centers, you will be working side by side with a team of 25 pharmacy technicians, a clinical pharmacist, and a pharmacy manager. You will be participating in daily operations of the pharmacy under the direction of the pharmacy manager. Our technicians are detail oriented and knowledgeable individuals with excellent customer-service, listening, math, organization, multi-tasking skills. Our pharmacy manager is a board-certified oncology pharmacist who has 17 years of pharmacy practice experiences in various areas such as inpatient, outpatient, adult medicine, pediatric medicine, and research.
Schedule: M-F day shift (no weekends or holidays). This position will work out of our Goodyear location and will travel between the additional locations in the West Valley.
Essential Duties and Responsibilities:
- Provides accurate, clinically relevant drug education to oncology physicians, nurses, and other care team members including advising on formulary chemotherapy and suggesting alternative therapeutic regimens/options as needed, performing drug utilization reviews, completing appropriate/accurate documentation.
- Optimizes the outcomes of oncology patients by providing evidence-based, patient-centered drug therapies and monitors for drug-related problems and medication cost containment.
- Serves as a resource for training and educating medical, nursing and pharmacy staff as it relates to medical oncology/chemotherapy.
- Identifies, prevents, reports and investigates potential adverse drug reactions and medication errors.
- Assists in the development and maintenance of a chemotherapy formulary and builds and reviews care plans.
- Supports regulatory requirements and local quality programs related to safe medication use.
- Review, process, and dispense medication orders after establishing appropriateness of drug choice, dosage, administration route and quantity in a timely manner.
- Keeps abreast of changing protocols and clinical pharmacy standards as they pertain to oncology.
- Ensures utilization of cost-effective medication therapy and patient access to treatment.
- Demonstrates accurate and thorough documentation skills in the electronic medical record. Facilitates pharmacist-to-provider communication of clinical interventions and recommendations.
- Adheres to clinical practices approved by the Pharmacy and Therapeutics Committee (e.g., collaborative practice agreements, clinical pharmacy protocols).
- Performs other duties as assigned by your department manager.
Qualifications:
- PharmD graduate of an ACPE accredited School of Pharmacy with a minimum of Doctor of Pharmacy degree or equivalent experience. Required.
- Registered Pharmacist (RPh) Licensed to practice pharmacy in the state of Arizona upon hiring. Required.
- Minimal of 1 year hospital pharmacy experiences. Required.
- Experiences in oncology pharmacy practice. Highly preferred.
- Completion of a PGY1 pharmacy practice residency (ASHP accredited) and three (3) years of clinical pharmacy experience OR a PGY2 oncology pharmacy residency. Preferred.
- BCOP (board certified oncology pharmacist). Preferred.
- Experiences in outpatient infusion center. Preferred.
- Experiences in medication management with oral oncolytic agents. Preferred.
