Job Title: Field Supervision Registered Nurse (RN)

Location: NYC

Job Type: Full-time

Job Summary:

We are looking for an experienced Field Supervision Registered Nurse (RN) to join a large Home Care Agency team! As a Field RN Supervisor, you will make initial and follow-up patient and home assessment visits to determine service needs, develop a Plan of Care, instruct, and supervise paraprofessional staff. You will also set up and update patient's care records after every follow-up visit. This position requires travel to different boroughs in NYC.

Key Responsibilities:

• Conduct comprehensive assessments of patients and families to determine home care needs
• Develop and initiate plans of care with patients' physicians
• Instruct, supervise and evaluate home health aide care
• Perform follow-up visits for recertification, staff supervision, and changes in condition
• Provide healthcare instructions to patients and families
• Communicate with physicians regarding patient needs and changes in condition
• Anticipate patient needs and recommend changes to physicians and families
• Participate in clinical on-call coverage and respond actively and timely

Requirements:

* Current RN license in NY
* 2+ years of experience as a Registered or Practical Nurse, with 1+ year of field supervisory experience
* Baccalaureate degree in nursing or associate degree with supervisory experience
* Proof of vaccinations and negative TB test
* Malpractice insurance and CPR certification
