Job Summary: The LCSW is responsible for conducting clinical consultations, evaluating and acting as a clinical resource to Lead Care Managers and management. 

Essential Functions: 

• The LCSW is responsible for providing support to LCM’s servicing adult and pediatric behavioral health members. 
• Conducts emergent clinical consultations for behavioral health members’ care management as needed. 
• Evaluates, adjusts, and conducts member Care Management Plan’s as needed. 
• Communicates recommendations, learning needs, and healthcare teachings to the Lead Care Manager and the member and documents notes in the member’s electronic health record (EHR). 
• Provide education to members and their families on health-related topics, treatment options, and self-care strategies, either in person or telephonically. 

• Assist with Discharge Planning: Help plan and coordinate the discharge process for behavioral health care members being discharged from inpatient psychiatric facilities.  
• Facilitates access to behavioral health providers, as needed. 
• Assists with New Hire Orientation training.  
• Coordinates, implements and monitors options and services to meet at risk members healthcare needs through communication and coordination with providers.  
• Providing individual and group supervision to ACSW’s employed with Titanium.  
• Understands and abides by all departmental and companywide policies and procedures. 
• Attends weekly Multi-Disciplinary Case Conferences and SCR and provides clinical recommendations.  
• Complies with all safety and injury prevention policies and regulations. 
• Performs other duties as assigned or required. 

Qualifications: 

• Education: Master's in Social Work  
• Experience: 3+ years’ experience as an LCSW in an acute care setting.  

• Experience with an EHR/EPM; eClinical Works (ECW) experience preferred. 
• Experience working in an internal medicine clinic preferred. 

• Language: Fluent in English (written and verbal). Bilingual preferred. 
• License/certification: Valid, unrestricted California state license.  

• Valid BLS certification from the American Heart Association. 

• Skills required: 

• Competent with computers, email, virtual platforms, Excel and other Microsoft Office based programs. 

• Excellent verbal and written communication skills, including the ability to convey and exchange information in a clear, effective manner. 
• Ability to identify problems and use logic and related information to develop and implement solutions. 
• Ability to work independently and carry out assignments to completion within the parameters of established policies and procedures. 

Physical Requirements: Physical demands associated with office work and driving, including but not limited to: 

• Consistently operating a computer and other office equipment such as a telephone, calculator, copy machine, and printer. 
• Frequently moving or carrying office equipment weighing up to 15 pounds across offices. 
• Ability to operate a vehicle and/or travel to meetings and different office locations as required or requested.
