RN Care Manager Details:

• Location: Fully Remote nationwide – MUST BE LICENSED IN CALIFORNIA
• Schedule: Monday-Friday 8:00am-5:00pm PST (occasional OT)

RN Care Manager Compensation

• The pay for this position is $35 - $42 per hour based on experience
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

RN Care Manager Qualifications:

• Active RN license in the state of California required
• Must have experience with Medicare / Medicaid / Managed care plans
• Care/Case Management experience
• Must have solid computer and documentation skills

RN Care Manager Responsibilities:

· Assisting with Backlog-referrals from CA-members sending referrals to case managers

· Develop, assess and adjust, as necessary, the care plan and promote desired outcome

· Assess the member's current health status, resource utilization, past and present treatment plan and services, prognosis, short- and long-term goals, treatment and provider options

· Coordinate services between Primary Care Physician (PCP), specialists, medical providers, and non-medical staff as necessary to meet the complete medical socio-economic needs of clients
