DESCRIPTION:


The Post Hospital Discharge Coordinator is the contact person for the inpatient case managers/coordinators to assist in scheduling all PCP and specialist appointments for qualified patients. In addition, the Coordinator will be outreaching to patients to schedule appointments and to remind patients of their upcoming appointments. The Coordinator must be very organized, be efficient, be professional and prompt when interacting with other departments and patients. The Coordinator is also detailed in obtaining medical records and tracking hospital discharges and appointments. Performs other duties as needed.

Schedule: 4 days per week, 10 hours per day


ESSENTIAL FUNCTIONS AND DELIVERABLES:


  • Processes incoming hospital discharges daily.
  • Reviews referral orders for accuracy of codes and supporting documentation.
  • Thoroughly, accurately and timely track hospital discharges to determine which patients are required to have appointments at the Post Hospital Discharge Clinics.
  • Interact with inpatient case managers and coordinators to assist in making appointments for qualified patients with all PCP & specialists post discharge.
  • Work with inpatient coordinator to help make reminder phone calls the day prior to appointments.
  • Review internal systems for medical records and upload them to the Electronic Medical Record to be accessed by the clinician during the appointment.
  • Assist in rescheduling appointments for patients who cancel or do not show to their scheduled appointments.
  • Maintain excellent tracking of project, process and scheduling.
  • Strive to meet personal, departmental and organizational goals and targets.
  • Ability to manage a heavy workload with frequent interruptions and changing priorities in a fast paced environment.
  • Completes assignments within specified timelines. Able to work well in a team environment on special projects.
  • Prepares and maintains records and files that are organized, thorough and readily accessible.
  • Attend regular team meetings and report findings of projects.
  • Maintains accurate and up-to-date tracking system for hospital discharges.
  • Monitors and reports productivity statistics data to supervisor on a weekly basis
  • When identifies a problem, informs supervisor in a timely manner so the problem can be corrected.
  • Transmits ideas and information in a clear and concise manner.
  • Develops and maintains professional working relationships with outside providers, vendors and insurance groups involved in the referral and discharge process.
  • Ability to work cohesively with all other SACHS departments.
  • Assists supervisor with special projects, as requested
  • Must be able to adhere to the Attendance policy and procedure.
  • Ability to collaborate, teach, support and function in a teaching organization that builds, coaches and embraces interns and residency programs. Must demonstrate a passion for the team and have the ability to consistently work with rotating doctors, residents and interns, while continuously building a trusting, safe, patient centered workplace.

QUALIFICATIONS:


  • Education: High School Diploma or equivalent required. Medical terminology required.
  • Licensure/Certification: N/A
  • Experience: Minimum of 3-5 years of outpatient referral or hospital discharge experience required and familiarity with medical insurance health plans, managed care, and point of service medical care. As a requirement of this position, you must receive EPIC certification for the module you have been hired into.
  • Essential Technical/Motor Skills: Computer literate with knowledge and experience with specific software such as Word, Excel, Access. Must have proficient computer skills including data entry (35wpm). Must be able to use a variety of office equipment including fax machine, scanner, printer and copier.
  • Interpersonal Skills: Skilled in working independently, but with supervision. Must have professional telephone etiquette combined with a pleasant and courteous voice. Fluency in English required. Must possess the ability to maintain a professional attitude in difficult situations and communicate effectively. Must perform as a team player.
  • Essential Physical Requirements: SEE PHYSICAL DEMANDS
  • Essential Mental Abilities: Requires critical thinking skills. Must be flexible to perform a variety of tasks. Must be well organized. Must be able to handle multiple telephone lines ringing simultaneously. Must be able to prioritize work using strong decision making skills.
  • Essential Sensory Requirements: Visual acuity to proficiently perform essential duties and view a computer screen for extended periods of time..



Must be legally authorized to work in the United States on a full-time basis. Must not now or in the future require sponsorship for employment visas.
