The Position:

The Medical Scribe will transcribe dictation from physicians and other health care staff. Dictation may include patient histories, diagnoses, medical treatments, analysis of test results, medications, prognoses, discharge summaries and autopsies.


Essential Responsibilities:

• Scribes accompany the provider upon patient interview and examination.
• Document the dictated patient history, including history of present illness, review of systems, past medical and surgical histories, family and social histories, medications and allergies.
• Document physical examination findings and procedures as performed by the provider.
• Learn about the medical process and using a team approach in patient supervision and documentation.
• Work with a supervising Physician or Doctor to thoroughly complete and submit medical records.
• Document the results of laboratory and radiographic studies as dictated by the provider.
• Take notes during patient visits and document them in the electronic health records system, if applicable.
• Verify patient information for accuracy.
• Prepare reports promptly and maintain an accurate filing system for records and charts so that they are ready when needed for future treatment.
• Document the correct time of patient care related activities, including provider to provider communication, family communication and re-examinations of the patient.
• Responsible to edit for clarity but is careful not to alter the meaning of the raw, dictated material.
• Maintain a reliable backup system to ensure that vital information will be preserved in case of a system malfunction.
• All orders for patient care must be communicated by the provider and not the Scribe.
• Other duties as assigned.


This job might be for you if:

• Must have the ability to hear and record medical reports accurately.
• Thorough knowledge of medical terminology, anatomy and physiology, medical procedures, medications, and laboratory test information.
• Competency with transcribing equipment and computer software for preparing reports.
• Confidentiality and privacy of the patients.
• Excellent command of the English language, grammar, and punctuation.
• Ability to edit when necessary without distorting meaning.
• Ability to work independently and to produce prompt, accurate reports.
• Prolonged periods of sitting at a desk and working on a computer.
• High school diploma, GED or equivalent.

• Certification by the American Association for Medical Transcription preferred but not required.
• Good finger dexterity to prepare reports.
• Good visual acuity to proofread reports.
• Ability to handle high-pressure situations effectively
• Good bedside manner and interpersonal skills

• MA certification or LPN licensure preferred.
