TB Nurse - RN Case Management

We are hiring a TB Nurse for one of our clients in Phoenix, AZ.

Job Description:

• Tuberculosis program RN Case Manager, manages the nursing needs of assigned TB patients. Coordinates with external healthcare organizations on patient care, provides direct patient care during patient appointments.
• Promotes health and wellness through direct contact with individuals, families, and groups.
• Provides health education, care management, and primary care to individuals and families who are members of vulnerable populations and high-risk groups.
• Works with communities or specific population groups to develop targeted health promotion and disease prevention activities.
• Provides consultation and technical assistance to community groups involved with vulnerable populations and high-risk groups.
• Provides direct patient care in the clinic, including patient data collection and documentation, specimen collection, tuberculosis screening and documentation, medication administration, and documentation.
• Provides immunizations, transport supplies, and vaccines.
• Provide direction for other patient services staff related to the provision of medical care.
• Maintain patient data, files, and correspondence with patients and healthcare providers.
• Performs other duties as assigned.

Skills Required:

• QFT & Blood draw Hearing and vision screening Electronic Health Record navigation and documentation Sputum collection Medication management Nurse Case Management State reporting and surveillance.

Experience Required:

• 3 or more years of RN experience Tuberculosis or other infectious disease experience preferred.

Education Required:

• Minimum Education and/or Experience: Graduation from an accredited school of Nursing.
• Training, Certifications, and Requirements: Valid, unrestricted Arizona Board of Nursing Registered Nurse License.

Work hours:

• 10:00 AM – 6:00 PM
