JOB DESCRIPTION – BILINGUAL COMMUNITY HEALTH WORKER

Position: Bilingual Community Health Worker

Reports to: Lead Supervisor of Social Services

Responsibilities:

The Community Health Worker is responsible for establishing positive and supportive relationships with residents, engaging and motivating residents to be active in their health and help develop health management care plans with residents. The Community Health Worker will work with the Support Service team to articulate and reflect the mission and philosophy of our program. This position includes but is not limited to the following list of specific tasks:

  • Conduct assessments of all assigned residents.
  • Utilize motivational interviewing and teach back approaches to ensure that residents understand their health issues and how to manage them.
  • Assist residents in understanding health care plans.
  • Help residents develop heath management plans and goals and follows-up with health management care plans with residents and providers.
  • Provide information to Northgate II residents about how to use the health care and social service system.
  • When appropriate and warranted, accompany residents to medical appointments.
  • Assess residents’ interest in evidenced-based programming and assist in enrolling residents in such programming.
  • Refer interested residents to internal and external health-related programming and activities.
  • Actively participate in care coordination and department meetings.
  • Meet individually with residents in their homes and in the social services office.
  • Impart concepts of health promotion and disease prevention, acting as a public health advocate.
  • Refer residents to a broad range of community resources.
  • Effectively work with people from diverse backgrounds (staff, residents, primary care physicians, agencies) in reducing cultural and socio-economic barriers between residents and healthcare related institutions.
  • Conduct research to identify culturally appropriate health intervention improvement methods. Uses these methods to effectively work with residents from different cultures.
  • Maintain confidentiality with health information.
  • Perform basic record keeping functions as needed including working with files, forms and documents.
  • Continuously expand knowledge and understanding of community resources, services and programs.
  • Identify and apply appropriate role definition and skilled boundaries.
  • Document activities, service plans and results in an effective manner while strictly adhering to policies and procedures.
  • Participate in ongoing health training.

Salary: $38,000/yr

Requirements:

Qualifications:

Incumbents must be:

Able to translate medical jargon into easily understandable lay language.

Able to establish and maintain positive and collaborative working relationships with residents, staff, and community providers.

Critical thinker and evaluative skills.

Excellent listening and assessment skills.

Knowledge of basic health information, interventions, and resources.

Able to effectively relate to individuals with diverse cultural backgrounds.

Bi-lingual (English/Spanish required).

Computer literate with a knowledge of Microsoft Word, Outlook, and Excel.

Knowledgeable of Camden healthcare system and community resources is a plus.

Remain professional in all aspects of the workplace.

Treat tenants with respect and dignity.

Pass a pre-employment background screening.
