Job Summary:

The Licensed Practical Nurse, LPN, will primarily provide routine care, observe patients' health, assist physicians and other healthcare providers, and communicate instructions to patients regarding medications and other treatments and functions of care.

Pay and Schedule:

Full Time / Day Shift

Starting at $30.19 - $36.84 depending on experience

Essential Functions:

• Assist in development of treatment plan for each elder and ensure the care strategy addresses the long term needs of the aging process
• Oversee other care staff at all times, ensuring the plan of care is being followed
• Advocate for elders best interests and serve as an educator to caregivers, elders, and families
• Work with other healthcare providers such as physicians and therapists to ensure high levels of quality care and services are always provided.
• Communicate any change in elder's condition to appropriate staff, health care providers, and family.
  
  

About the Green House Model of Care:

The GH Model creates "caring homes for meaningful lives" for elders where residents have private rooms and baths, can move freely through the home, build deep knowing relationships with each other more and even participate in preparing their own meals. It is based on a philosophy seeking to reverse the "enforced dependency" of life in a traditional nursing home by creating small intentional communities of 10 to 12 elders designed to foster late-life development and growth. Residents of Green House Project homes have shown "increased reports of mobility and social interaction, and fewer reports of weight loss and depression" compared with those living in traditional nursing home facilities.

Minimum Qualifications:

• Must be able to work in the federal health program.
• Must be Licensed Practical Nurse listed on the MN Board of Nursing Registry
• CPR/First Aid
  

Preferred Qualifications:

• Long Term Care Experience
• Experience with electronic medical records
