Preventative Aftercare was established in 1993 and is a community-based program.

Summary of the position: The Mobile Medication nurse is responsible for the overall operation of the Mobile Medication Team.

Description of Duty

Consumer Services

  • Interact with consumers in a respectful manner that is appropriate for the situation.
  • Participate in weekly team meetings.
  • Complete initial nursing assessments and on-going assessment required in the implementation of consumer care. Develop comprehensive treatment plans for those served to address physical and behavioral health needs using recovery tools and principles.
  • Address physical health care needs of consumers and take necessary action to resolve identified needs, including appropriate referrals to primary care.
  • Develop, supervise, and review consumer progress in self administration of medication. Consult with treatment team to develop transition plans to traditional services as consumers complete treatment goals.
  • Ensure appropriate medication administration and compliance with agency nursing policy regarding medication management. Administer medications as prescribed for consumers following the scope of nursing practice standards.
  • Provide training to clients regarding health and hygiene methods, infection control procedures, and signs and symptoms of illness.

Collaboration

  • Keep communication open with all levels of staff.
  • Serve as the program contact person with the pharmacy services used by consumers in the program, resolving issues with pharmacy services.
  • Develop and maintain positive working relationships with referring agency staff, PCPs, hospitals, psychiatrists, and MCO staff.
  • Consult, network, and cooperate with community systems to facilitate linkage, referrals, crisis management, advocacy, and follow up with the focus on attaining treatment goals.
  • Interact with clients and their supports along with non-agency personnel in a professional manner.

Documentation and Records Management

  • Document all consumer and collaborative contacts in a timely and professional manner.
  • Collaborate with Mobile Med RN Program Manager to ensure that all insurance is verified prior to starting service, that initial and continued stay authorizations are obtained, and that clinical documentation shows consumer meets medical necessity criteria.
  • Maintain consumer files and program records ensuring that all required documents are in accordance with applicable standards and regulations at all times.
  • Maintain all consumer data, outcomes and follow-up information accurately and update as required.

Referrals

  • Ensure that all referrals are processed promptly and that referral forms are completed properly.

Compliance

  • Ensure that all case records, correspondence and conversations regarding consumers remain confidential as required by HIPAA compliance mandates.
  • Comply with all Mandated Reporting, Duty to Warn and Duty to Protect Laws in all professional work.
  • Prepare for and participate in scheduled Quality Assurance Reviews of data, reports, consumer files, paperwork, checklists, etc.
  • Participate in monitoring quality initiatives in the department.
  • Ensure that incident reports are thoroughly completed and filed in a timely manner.
  • Maintain expectations for average hours of face-to-face time per week and use of Consumer Recovery Library.

Coordination of Treatment

  • Maintain regular contact with Mobile Med RN Program Manager, returning all calls and e-mails in a timely manner and being available for regularly scheduled clinical supervision.
  • Attend and actively participate in all scheduled professional trainings.
  • Be available to consumers, outside agencies, and co-workers via cell phone and return all calls within a timely manner.
  • Monitor all electronic communications throughout the day and respond accordingly. Ensure electronic communications reflect days off with instructions on whom to contact in your absence

  • Apply cultural sensitivity in clinical and professional work, including regular participation in the development of cultural awareness.
  • Work as a member of a team to include being professional, supportive, and friendly to co-workers; keeping personal problems away from work; assisting in duties as part of an interdisciplinary team; assisting co-workers and peers when requested; and welcoming new employees giving them extra support and encouragement.
  • Provide a professional role model for staff and consumers in appearance, actions, manner of speech, etc.
  • Follow through on directives given by the supervisor or Senior Management staff.


Working Conditions

Lifting Requirements

Must be able to lift up to 10 pounds.

Physical Requirements

Must be able to bend, lift and stretch.

Work Location: This position works in the community.


REQUIREMENTS

Education

Bachelors of Science in Nursing preferred but not required

Licensure

Current unrestricted Pennsylvania Registered Nursing or Licensed Practical Nurse License

Years of Experience

Minimum of:

  • 2 years nursing experience
  • 1 year psychiatric nursing experience serving seriously mentally ill adults strongly preferred

Computer Experience

Basic skills in Microsoft Word and Excel

Other

  • Act 33/34 clearances as provided by Child Protective Services Law 23 PA C.S. 16301 and FBI Clearances, Adam Walsh Act 147
  • Valid Driver’s license.
  • Auto insurance coverage for $300,000.
