MINIMUM EDUCATION & EXPERIENCE
· High School diploma or equivalent
· Computer literate
· 6 months - 1 year of experience filing and pulling of charts according to numerical and color coding
· 6 months of medical records experience (preferred)
· Familiarity with copier machines
· Knowledge of confidentiality of records (HIPAA)
· Basic knowledge of how to communicate with mentally ill clients (highly desirable)
· Good communication skills
· Bilingual - French Creol (favorable)
RESPONSIBILITIES
FILE AND PULL CHARTS FOR AUTHORIZED CENTER PERSONNEL
· Pull the charts for Med Clinic, Drs., Nurses and Lab.
· File all charts back in main file system.
· Keep charts and file system ready for the following day.
FILE LOOSE RECORDS AND NRC RECORDS
· File loose records from screening, CSU, CCSU, SRT, Detox, Re-entry, and traditional Case Management.
· File all Non-Registered Clients Records (NRC Records).
DATA ENTRY
· Log in and out charts removed from medical records department on computer program
COPY RECORDS
· Respond to all requests for records from authorized external agencies.
· Copy, stamp, bill and send copies out.
· Enter every request in the computer which is sent out.
MAKE NEW CHARTS
· Open a new chart for new clients.
· Prepare new chart on every new client.
· Assemble all the different dividers and loose papers in chart.
OTHER MINOR OR OCCASIONAL TASKS
· Pull the chart for Interim Status to Discharge Status for volunteer or supervisor.
· Print the forms for restocking of the supply of all forms.
· Help each staff member who needs help with the medical records and other items under the department’s responsibility.
· Answer the telephone for any internal and external calls for information.
· Pull and make a list of charts that need to be sent to storage by putting in a box marked with chart numbers and box numbers.
· Shredding of all confidential records.
· File all interim and discharge records.
· Perform other duties as assigned.
