Job Summary:

The position of Case Manager will be responsible for providing services to Logan Height's Youth Prevention and Intervention clients and the community. The Case Manager will ensure to keep up with Logan Heights CDC goals, objectives, mission, and style and will provide a team framework. Additionally, this role will work with the Logan Heights CDC internal and external partners to develop and meet team goals and objectives.


Duties and Responsibilities:

  • Complete comprehensive family-based assessments for identified youth, including the Family Wellbeing Assessment and the Strengths and Difficulties Questionnaire;
  • Provide family-centered case management services, and other services relative to program goals and objectives agreed upon with the client based on the Assessment;
  • Develop strength-based, individualized family treatment plans including goals and objectives;
  • Conduct home visits, school visits, and field visits, in addition to office visits, to best meet the needs of the youth and family;
  • Assist in the planning, coordination, development, monitoring, and evaluation of services for youth and families with emphasis on quality of care, continuity of services, and cost-effectiveness;
  • Maintain case records and client files in the Efforts to Outcomes (ETO) database as required by SBCS and its funding sources;
  • Facilitate weekly evidence-based, trauma-informed center-based groups for youth and/or caregivers;
  • Outreach to target population to effectively reach those in need of services;
  • Participate in special projects and activities as related to the agency's goals and objectives;
  • Participate in agency special events, such as fundraisers, annual meetings, and open houses;
  • Participate and assist in implementing program components;
  • Conduct crisis intervention services, and provide referrals to telephone and walk-ins;
  • Participate in Multi-Disciplinary Team meetings with other team members;
  • Coordinate with contacts on other public and private organizations and individuals, as necessary to effectively serve clients. (e.g. progress reports to referral sources, client referrals to other agencies); and
  • Using ETO and Salesforce, provide monthly statistical reports of services to SBCS as needed.

Qualifications/Requirements:

  • Ability to work outside of the normal working hours such as early mornings, late evenings, and weekends;
  • Ability to travel to various locations;
  • Attending conferences or training sessions within [county] and in- or out-of-state locations;
  • Bachelor's degree in Psychology, Sociology, Child Development, Criminal Justice, Social Work, or related field is required;
  • A minimum of one (1) year of experience working in child, youth, and family services is required;
  • Experience working in individual, group, family counseling, school-based services, outreach, community education and development, and or advocacy and case management is preferred; and
  • Experience working in community-based or neighborhood service organizations is a plus;
  • High proficiency in MS Office, including Word, Excel, Outlook, PowerPoint, and SharePoint; and
  • Working knowledge of all standard office equipment such as computers, printers, and telephones.
