Job Description: FUNCTION: Provides, plans, coordinates or manages nursing services and health education to residents requiring complex care. QUALIFICATIONS: The requirements listed below are representative of the knowledge, skill and/or ability required. Minimum Qualifications: • A registered nurse with management or supervisor experience in long-term care or geriatric nursing. • Current and unencumbered state license to practice as a nurse. • Current CPR certification and additional certification in nursing specialty desired. • Demonstrates ability to read, write and speak English sufficient to fulfill job requirements. • Demonstrates the ability to make independent decisions, solve practical problems that may or may not include a variety of concrete variables. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES: To perform this job successfully, an individual must be able to perform each key function satisfactorily. 1. Plans, organizes and evaluates the care for residents requiring complex medical care, complex skilled nursing care and physical assessment. 2. Creates, reviews and implements systems for addressing the unique needs of residents with complex medical and complex skilled nursing needs. 3. Manages the care for residents dependent upon medical equipment such as infusion therapy, enteral feeding pumps, ventilators and monitors, 4. Prevents delay in resident’s recovery by recognizing and assessing changes in resident’s physical condition early and notifies the physician or licensed independent in a timely manner. 5. Uses a systematic approach to provide individualized nursing care. 6. Performs nursing assessments regarding the health status of the resident. 7. Contributes to the resident’s assessment (MDS/RAP’s) and the development of a plan of care. 8. Makes nursing diagnoses, which serve as the basis for the strategy of care. 9. Develops a plan of care based on assessment, implementing nursing care. 10. Evaluates the resident’s responses to nursing interventions. 11. Selects and institutes appropriate nursing intervention which might be required to stabilize a resident’s condition and/or prevent complications. 12. Clarifies any order or treatment regimen believed to be inaccurate, non-effective or contraindicated consulting with the appropriate licensed practitioner and notifying the ordering practitioner when making the decision not to administer medication or treatment. 13. Accurately reports and documents the resident’s symptoms, responses and status. 14. Demonstrates measures to promote a safe environment for residents and others. 15. Designs, promotes and organizes resident’s education and counseling based on health needs. 16. Collaborates with the resident, members of the healthcare team and, when appropriate, the resident’s significant other(s) in the interest of the resident’s healthcare. 17. Consults with, utilizes and initiates referrals to appropriate community agencies and healthcare resources to provide continuity of care. 18.Monitors residents who self-administer medications and report deviation frocare to the licensed or registered nurse. 19. Uses safe and proscribed procedures for medication administration.Identifies residents before administering medications. 20. Reports a drug administration error or drug reaction to the charge nurse and completes report as appropriate. 21. Removes discontinued, outdated or deteriorated drugs from inventory. 22. Reviews medication and treatment records for completeness, accuracy in transcription of physician orders and adherence to stop order policies. 23. Administers and documents administration of medications, enteral nutrition and treatments per the physician’s order and accurately records all care provided. 24. Orders and documents receipt of medications. 25. Supervises the serving and documentation of prescribed diets and fluid intake. 26. Counts al narcotics, signs for count and exchanges keys to medication cards and medication room. 27. Checks emergency equipment (oxygen and suction) and emergency medication kit if application). 28. Follows procedures related to the use of nursing equipment and supplies. 29. Performs other duties as directed. Maintenance Of Supplies/Equipment: 30. All equipment is operated in a safe manner and the only equipment utilized is that which previous training of use has occurred. 31. Defective equipment is reported to the Manager. 32. Utility room is checked routinely for supplies that need to be returned to the appropriate department, meal trays requiring return to dietary, and linen that requires removal to the laundry. 33. Safety hazards are removed from unit floors (liquid spills, food, paper, equipment cords, etc.). 34. Demonstrates knowledge of and compliance with Infection Control/Universal Precaution/Blood Borne Pathogen, OSHA, HIPAA and confidentiality policies. 35.Safety guidelines established by the facility (i.e. proper restraint and side-rail use) are followed. 36. All accidents or incidents observed are reported on the shift that they occur. 37. All changes in a patients/residents condition are reported as soon as possible to the appropriate parties. 38. Staff meetings are attended, if on duty. Minutes read and initialed if not on duty. 39. Resident/unit problems are reported to Supervisor/Manager. 40. Decisions are made that reflect both knowledge and good judgment. 41. Awareness of own limitation is evident and assistance is sought when necessary 42.Committee meetings (if assigned) are attended. Reports related to the committee are given during staff meetings. 43. Responsibility is taken for own professional growth. Attend and participate in all scheduled training, educational and orientation classes, programs and activities including annual mandatory classes. 44. Follow work schedules and complete assigned tasks according to the established policies and procedures of the facility. 45.Complies with Attendance/Tardiness policy. 46. Organizational ability and time management is demonstrated. 47. Cooperate with inter-departmental personnel to ensure that the needs of the residents are met and maintained. Human Relations: 48. Cultivate and demonstrate a positive working relationship with residents, visitors and facility staff. 49. Authority is acknowledged, and response to the direction of supervisors is appropriate. 50. Time is spent with patients/residents rather than other personnel. 51. Co-workers are readily assisted as needed. 52. Participate as requested by the Medical Director or Director of Nursing Services in the planning and conducting of in-service training classes concerning dermatologic nursing skills and procedures used within the facility. 53. Participate in educational programs for residents and their family members relative to skin care when a resident is being discharged to home. 54. Attend and participate in required annual in-service programs for hazard communication, TB management, bloodborne pathogens standard, advanced directive, and other related programs. 55. Be familiar with and use as appropriate all items of personal protective equipment offered by the facility, including, but not limited to, masks, gowns, gloves, and back brace to be worn when lifting. 56. Wear and/or use safety equipment and supplies (e.g., back brace, mechanical lifts, etc.) when moving or lifting residents. 57. Maintain the confidentiality of all resident care information. 58. Monitor all care and activities of residents to ensure that residents are treated fairly, and with kindness, dignity and respect. that all nursing care is provided in privacy and that nursing service personnel knock before entering the resident’s room. 59. Report any complaints or grievances made by residents regarding nursing or medical care to the Director of Nursing Services. Maintain a written record of any resident’s complaints or grievances communicated to you with a notation of actions taken. 60. Report and investigate all allegations of resident abuse and/or misappropriation or resident property. 61. Ensure that nursing staff personnel honor the resident’s refusal of treatment request. Ensure that such requests are in accordance with the facility’s policies governing advance directives. 62.Functions independently with integrity, flexibility and works well with residents, personnel and support agencies. 63.Demonstrates knowledge of Employee Handbook and complies with the policies in the Handbook. 64.Performs other related duties as may be assigned. PHYSICAL Requirement: (% of work time) Seeing: Must be able to read reports and use computer. Hearing: Must be able to hear well enough to communicate with co-workers, residents. Standing/Walking/Mobility: Must be able to reach above his/her head and perform repetitive movements on feet over long distances. Climbing/Stooping/Kneeling: Must be able to bend to the floor. Lifting/Pulling/Pushing: Must be able to transfer up to 50 pounds. Fingering/Grasping/Feeling: Must have finger dexterity to open containers. WORK ENVIRONMENT: While performing the duties of this job, the employee is occasionally exposed to blood or other body fluids, fumes airborne particles and toxic or caustic chemicals.
