Job Description

The core function of the re-entry case manager is to lead the development, implementation, and tracking of re-entry plans for inmates of Cumberland County Jail. The re-entry case manager will facilitate client access, utilization and follow through with treatment, social services, medical, vocational, housing, peer/social support, family-based support, and community resources. This position requires strong collaboration and coordination with other program partners inside and outside of the jail. The re-entry case manager is supervised by the Cumberland County Sheriff’s Office and works in collaboration with the Public Health Department.   This position is 40 hours per week, with a full benefits package.  Pay is $24.09 per hour.  Position will remain open until filled.  Please note, applicants must be authorized to work for ANY employer in the U.S. We are unable to sponsor or take over sponsorship of an employment Visa at this time.

QUALIFICATIONS: 

  • Valid driver’s license.
  • High School Diploma required; bachelor’s degree or higher in social work or related field strongly preferred.
  • Substance Abuse, LCSW, LCPC or other equivalent certification and/or licenses strongly preferred.
  • Experience with case management of mental health, substance misuse, and homeless populations in a corrections setting preferred.
  • Equivalent combination of education, training, and experience will be considered.

MAJOR RESPONSIBILITIES:
1. Lead the development, implementation, and tracking of re-entry plans, including:

  • Coordinate with other staff and program partners to communicate and utilize the results of intake screenings and assessments to inform triage and re-entry planning. 
  • Meet with clients on a regular basis to develop and review re-entry plans including identifying needs, goal setting, and supporting the implementation of individual objectives and interventions.  
  • Actively participate in case review, case conferencing, and program evaluations. 
  • Collaborate with clients, jail, medical, and community partners to coordinate services and support participants in service engagement, retention and goal achievement.
  • Monitor and track re-entry plan implementation and respond progressively to need as the client continues to engage in the community post discharge.
  • Complete all required forms, case notes, data collection, recording activities and reports necessary for effective case management and project accountability.
  • Case notes should be entered no more than 48 hours after contact. 
  • Attend staff and supervisory meetings and other meetings as scheduled.
2. Complies with Federal, State, Local laws, and accreditation policies and procedures by actively seeking material such as books and articles and settings (workshops) in terms of updating knowledge and skill in the human services field.
3. Stay up to date on evidence-based practices in the re-entry field, especially for people with substance use disorder, and make programmatic recommendations and quality improvements. 
4. Supports the Sheriff’s safety and health policies by attending required safety training programs; reporting all accidents, and suspected safety hazards to supervisor.
5. Fulfills job function by participating as an active member in training, participating on county task forces and performing other responsibilities as assigned. 
6. Develop relationships with community resource leaders to provide services to inmates while incarcerated and once released into the community.
7. Facilitate meetings with multiple jail and community partners.
8. Performs similar or related work as required, directed or as situation dictates.
