Title: Clinical Lead Care Manager - ECM
Base Salary Range: $131,461-$152,315 plus benefits
FTE: Part-Time Non-Exempt (16-20 Hours)
Responsibilities: We are seeking a dynamic and experienced Clinical Lead Care Manager to join our Enhanced Care Management Program. As the Clinical Lead Care Manager, you will play a pivotal role in overseeing and coordinating care management activities, ensuring the delivery of effective and efficient healthcare services to our patients. This role requires a combination of clinical expertise, patient advocacy, and a passion for improving patient outcomes.
- Care Coordination: Coordinate and facilitate care for patients with complex medical needs, ensuring seamless transitions between healthcare providers and settings.
- Patient Advocacy: Advocate for patients' needs and collaborate with the interdisciplinary healthcare team to address barriers to care and improve patient outcomes.
- Quality Improvement: Collaborate with the ECM Administrator to develop and implement initiatives that enhance the quality and efficiency of care management services.
- Training and Development: Provide ongoing training and professional development opportunities for care management staff to ensure competence and compliance with program goals of health promotion and connection to clinic.
- Documentation in NextGen EHR System: Ensure accurate and timely documentation of client interactions in the NextGen Electronic Health Record (EHR) system, maintaining compliance with LA Care, Health Net and Molina Healthcare reporting standards.
- Health Promotion Material Development: Source, create, and disseminate health promotion materials for Lead Care Managers to distribute to program members. Collaborate with internal and external stakeholders to ensure materials align with evidence-based practices and cultural competence.
- Willingness and ability to manage a small caseload (1-10) of high-needs clients as needed.
Education and Knowledge:
- Bachelor’s degree in nursing, social work, or a related field. Master’s degree preferred.
- Knowledge of or willingness to learn LA Care, Health Net, and Molina Healthcare reporting requirements for clinical oversight.
- Must have a minimum 1-3 years experience in a community clinic or FQHC setting
- Experience working with patients with limited literacy and from socioeconomically disadvantaged backgrounds.
- Bilingual and Bi-literate Spanish/English required.
Licensure and Certifications
- ·Valid Registered Nurse (RN) license in California or Psychiatric Mental Health Nurse Practitioner (PMH-NP) or PMH Clinical Nurse Specialist (PMH-CNS) or Advanced Practice Nurse or Nutritionist or Social Worker.
