Program: Community Oriented Recovery and Empowerment Services (CORE)

Hours Per Week: 35

Location: Tarrytown, NY (Mobile)

Reporting Structure: Program Director

Summary Description: The Psychosocial Rehabilitation Specialist works with Medicaid beneficiaries with behavioral health conditions and substance use disorders (SUD) through the Community Oriented Recovery and Empowerment Services (CORE) program. This role utilizes a recovery model of care that empowers individuals to improve their quality of life and reduce symptoms by providing services in their own homes or communities. The Specialist supports clients on their personal recovery journeys, fostering hope, self-direction, and social inclusion while guiding them toward their goals in education, employment, housing, and overall well-being. Through a person-centered approach and collaboration, the Specialist plays a key role in helping clients integrate into their families and communities.

Key Responsibilities:

  1. Conduct psychosocial assessments to identify individual strengths, needs, and goals.
  2. Develop and implement individualized rehabilitation plans in collaboration with program participants.
  3. Provide emotional support and encouragement to individuals in their recovery journey.
  4. Advocate for program participants in accessing community resources and support services.
  5. Monitor and document participant progress and adjust rehabilitation plans as needed.
  6. Organize and participate in community outings and activities to promote socialization and community integration.
  7. Provide crisis intervention and support to program participants as needed.
  8. Implement person-centered, trauma-informed, and motivational interviewing practices in line with recovery-focused care.
  9. Promote recovery by embracing a model that includes self-determination, choice, shared decision-making, and collaboration.
  10. Restore individual functional levels for integration into family, community, and culture with minimal ongoing professional intervention.
  11. Other Duties as assigned

Key Competencies:

  1. Prior experience with trauma-informed, person-centered practice, and motivational interviewing.
  2. Understanding the recovery process and ability to facilitate recovery using standardized mental health processes.
  3. Ability to teach and engage in basic problem-solving strategies to support self-directed recovery.
  4. Knowledge of common challenges associated with mental health conditions.
  5. Knowledge of electronic record systems, with strong organizational skills and willingness to adapt and grow.

Key Performance Indicators (KPIs):

  1. Effectively restoring clients' functional levels and promoting their integration into family, community, and culture.
  2. Timely completion and management of electronic records and related documentation.
  3. Demonstration of knowledge and use of evidence-based practices in treatment and recovery.
  4. Client satisfaction and progress toward personal goals.

Requirements:

  1. Must be at least 18 years of age and have a high school diploma or equivalent, with 1-3 years of relevant experience working with individuals with SUD disorders and/or behavioral health conditions or a BA degree.
  2. Certification as a Certified Peer Specialist/Certified Recovery Peer Advocate or equivalent education in the human services field, with one year of education substituting for one year of experience.
  3. Basic computer knowledge.
  4. Reliability and ability to work independently while maintaining confidentiality.
  5. Valid driver's license with satisfactory driving record.
  6. Current New York State Defensive Driving certification (if driving agency vehicles).
  7. Clearance from the NYS Justice Center through fingerprinting for a criminal history records search.
  8. Clearance from the NYS Office of Children and Family Services for instances of child abuse and/or neglect.
  9. Legal eligibility to work in the United States.

Special Considerations: Weekly work schedule may include evenings and weekends. This is a mobile community-based position.
