POSITION DESCRIPTION & OVERVIEW

Title: Housing Support Coordinator - (Internal)

Reports To: Contracts Manager, Housing Programs

Schedule: Mon-Fri, Daytime Flexible

Compensation: $51,000 - $55,000; DOQ

Job Classification: Full Time, salaried Exempt

The Housing Support Coordinator is a key position within the Contracts and Compliance Department at Agate Housing and Services. This role is responsible for managing all compliance and financial management activities related to housing support programs for families and single adults, involving communication with funders as well as working closely with case managers. Agate's Housing Support program is primarily scattered site with funding to serve approximately 400 households in Hennepin County.

PRIMARY DUTIES AND EXPECTATIONS

Program Compliance (45%):

  • Review participant files to ensure compliance with program requirements.
  • Ensure accurate verification of participant and unit eligibility, as well as documentation of required county and state paperwork.
  • Monitor annual certification dates for participants as required by program funders. Prepare and mail out relevant paperwork and notices to participants and their case managers.
  • Coordinate client referrals and intake paperwork for the housing support program alongside Supportive Housing Team Leads.
  • Coordinate with Agate Housing and Services program staff and housing locators to ensure a habitability inspection is completed and passed before move-in and before payment starts.
  • Record and track Assistance Letters for participants which confirm their eligibility for supportive services.

Coordination with Program Staff (25%):

  • Work closely with Agate Housing and Services program staff to ensure documentation for all income or housing changes is completed. Provide 30- and 60-day notices for recertification to staff and clients.
  • Coordinate with program staff and HMIS Data Specialist to ensure timely entry of all paperwork and updates in ClientTrack so that the HMIS Data Specialist can make changes in the HMIS system and ensure a high-level data quality on all reports.
  • A small portion (<20) of Housing Support Clients are enrolled in a program called 'Combo' through which they receive an additional source of rental funding. The Housing Support Coordinator will have a monthly check-in with the Rental Assistance Coordinator to ensure all records and payments for these clients are in order.

Financial Compliance (20%):

  • Manage and track monthly rental payments and secondary check requests.
  • Coordinate with Contract Accountant to ensure monthly reconciliations and payment received are correct. Follow up on any missing payments.
  • Manage the 'Agate Store' budget which provides household needs and hygiene products for households in the Housing Support program.
  • Provide regular program budget updates and tracking to the Supportive Housing Managers and leadership teams.

Contract Management & Funder Relations (10%):

  • Maintain professional working relationship with Hennepin County and DHS staff to ensure documentation is up-to-date and the program is in compliance with funder requirements.
  • Co-submit applications for housing support funding and contracts alongside Contracts Manager.
  • Coordinate funder audits and collect necessary documentation from Housing Programs case files and accounting records.

PHYSICAL REQUIREMENTS FOR THIS POSITION

  • Sedentary work - Sitting most of the time with occasional walking.

REQUIRED QUALIFICATIONS

  • At least two years of experience working with landlords, subsidies, rental management, or in a related field.
  • Strong knowledge of Microsoft Word and Excel.
  • Exceptional organizational skills and attention to detail.
  • Strong verbal and written communication skills.
  • Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds or people in crisis situations.
  • Experience with de-escalation and conflict resolution
  • Ability to pass criminal background check required for working within a DHS-funded program.
  • Fluency in languages used by our clients (Spanish, Amharic, Oromo, Somali, ASL) preferred.

REFERRAL BONUS PROGRAM

To be eligible for rewards, candidates must be hired as permanent full or part-time employees (this excludes on-call, employees, temporary employees, interns or contract workers).
