Job Description
This position contributes to AccentCare's mission by overseeing compliance and regulatory requirements, accreditation standards, monitoring and improving quality care through collaboration with program site leadership and other regional leaders to ensure training, monitoring and improvement activities are designed an implemented to enhance quality of care provided by hospice professionals and all comply with regulatory requirements and accreditation standards.

1. Collaborate with site leadership team(s) to:
  • Participate in planning, implementation, evaluation and monitoring of all Quality Assessment & Performance Improvement (QAPI) activities
  • Assess educational and training needs of program site staff by referencing Adverse Event trending analysis,QI dashboard, triage trending, documentation review, and survey result review
  • Ensure maintenance of Quality binders on site for QAPI, Infection Control, Emergency Preparedness, Performance Improvement Projects (PIP), and Adverse Events
  • Provide education on quality improvement initiatives, documentation best practices, and compliance with regulations and Joint Commission (JC) standards
2. Conduct medical record audits for compliance with state/federal regulations, JC standards, and overall quality of documentation
 
3. Conduct administrative audits and internal surveys of personnel records and contracts for compliance with state and federal regulations and JC standards
 
4. Assist program sites during State and/or Medicare or JC surveys, and in the development and execution of corrective
action plans
 
5. Assist sites in preparing for an initial JC survey and with the annual Intracycle Monitoring
 
6. Report to the Chief Compliance Officer-VP-Compliance matters related to compliance and any resulting corrective
action related to government audits or investigations
 
7. Assist in identifying high-risk AEs and ensuring the AE is resolved according to policy and regulatory requirements
 
8. Serve on the QAPI steering committee and, as requested, on the Corporate Compliance Committee
 
9. Coordinate with leadership to ensure QAPI program meets federal, state and local regulatory and accreditation standards for quality management
 
10. Ensure timely completion of annual QAPI plan or framework, Infection Control plan, and Emergency Preparedness plan
 
11. Ensure QAPI meetings are held, and ongoing QAPI activities are analyzed, discussed, and reported bi-weekly

12. Support site leadership:
  • To prepare for and lead quarterly QAPI meetings with field staff participation
  • To monitor patient care and/or staff problem areas and selecting appropriate PIP
  • Supports PIP teams and guides implementation of solutions for local and national PIPs.
14. Provide additional support to sites as needed
 
15. Participate in Eligibility process
 
16. Other duties as assigned
