Job Description

We are seeking a Certified or Licensed Social Worker to join our adult day care team in Fort Lee! You will provide emotional and administrative support to clients in either individual or group settings.

Responsibilities:

Assess nature of clients' situations by interviewing and reviewing personal history
Develop and execute individual treatment or care plans
Determine appropriate milestones to gauge client progress
Communicate client updates to all relevant parties
Facilitate referrals to other healthcare professionals and programs
Maintain accurate client documentation
Arrange individual or family and group counseling as needed
Participate in the day care's quality improvement program, professional organizations and seminars
Document assessments, plans, evaluations and notes

Qualifications:

Experience in social work, counseling, or other related fields is a plus, but willing to train!
Compassionate and caring demeanor
Ability to build rapport with clients, notably seniors
Strong leadership qualities
Excellent written and verbal communication skills
