Job Description
Salary:
Job responsibilities can include but are not limited to:
- Asses patient/family to identify spiritual needs and develops a spiritual plan of care.
- Collaborates with local clergy as needed.
- Conducts services of prayer, worship and ritual for patient/families, as appropriate.
- Provides individual patient/family counseling, crisis intervention, information and assistance with preparation of advanced directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
- Work with IDT, participates in the ongoing POC by developing, documenting and evaluating responses to interventions in order to meet spiritual needs of the patient and family.
- Provides consultation, education, and support to the interdisciplinary team on spiritual care and bereavement counseling.
- Makes visits to patient/family prior to death and will be available to family during the bereavement period as needed.
- Comply with applicable legal requirements, company standards, policies and procedures
Qualifications:
- Qualified candidates will have at least one year of experience as a Chaplain in a hospice setting
- Masters of Divinity preferred
- Must have a current Virginia Driver's license and dependable transportation
