Job Description: Job Summary: This position is responsible for providing integrated, comprehensive pharmacy services to provide the highest quality pharmaceutical care to Oyate Health Center (OHC) relatives. Essential Functions and Professional Behavior Expectations: Communication and Teamwork: Serve on an integrated team to provide comprehensive and innovative pharmacy services as the primary drug therapy resource. Focus on a relationship-based, relative (patient)-centered approach to integrated care. Serve as a primary liaison between clinic and outpatient pharmacy. Work with facility, medical and nursing staff, support staff, and clinic and facility administration where necessary to resolve issues, incorporate clinic services seamlessly into site workflow, and to continually demonstrate the value of the ambulatory clinical pharmacy program. Work with the relative to identify goals of therapies that will maximize effectiveness and satisfaction. Provide relative with appropriate educational materials and information in order to help them understand and actively participate in their individualized care. Independent decision-maker that maintains a clinical pharmacy practice: self-starter that proactively takes initiative. Works directly with the Director of Pharmacy to coordinate, improve, expand, and deliver optimal ambulatory care pharmacy services including comprehensive IV services. Share and receive information, opinions, concerns, and feedback in a friendly manner. Work collaboratively by building bridges and creating rapport with team members within departments and across the organization. Technical: Prescreen relatives focusing on polypharmacy and hospital discharge interventions. Coordinate documentation retrieval between pharmacies in advance of relative arrival to appointments. Identify preventative and health maintenance issues. Provide medication reconciliation interview with relative to include allergy assessment, over the counter and outside pharmacy use, medication history, compliance, and dosing. Ensure any questions pertaining to drug therapy are answered including information on drug interactions, side effects, dosage, and storage of pharmaceuticals. Clean-up the relative’s active medication profile; follow and adjust developed treatment plans; analyze medication regimens; document all clinical activities and update medical information including utilization of SOAP notes; order and/or interpret laboratory tests for medications which require monitoring; titrate or alter medications as appropriate; consult with medical providers as required. Input, screen, process, and/or dispense prescriptions taking the following into account: drug interactions; drug duplication; therapeutic outcomes; drug-disease interactions; laboratory values; and compliance. Design a comprehensive drug therapy and education plan for relative-specific problems and persuasively justify recommendations based on relative-specific pharmacologic, pharmacokinetic, pharmacodynamic, pharmacogenomic, pharmacoeconomic, ethical, legal, and evidence-based information. Assesses medication therapy regimen for appropriateness, effectiveness, tolerability, and safety. Properly utilize the Electronic Health Record (EHR), ScriptPro, BD Pyxis, Converge, iCARE, and Consensus. Enter outside medications into chart to promote continuity of care and comprehensive data availability. Provide technical leadership of the pharmacy’s automated systems when designated. Serve as clinical lead for pharmacy residency program preceptorship and/or residency directorship. Responsible for pharmacy clinical coordination and oversight including planning protocol development and review, organization, implementation, maintenance, and assessment of a collaborative practice agreement disease management clinic. This includes the coordination, facilitation, education, relative care, testing, assessment, monitoring, and reporting of health promotion/disease prevention programs where drugs can be initiated, adjusted, and/or discontinued. This may include, but is not limited to, the following: diabetes, hypertension, dyslipidemia, anticoagulation, hypothyroidism, mental health, pain, spirometry, asthma, smoking cessation, vitamin D, and contraception. Arrange schedules for the arrival of students and ensure proper paperwork and training has been completed. Mentor pharmacy students on chart review, medication/case management presentations, and counseling techniques. Instruct medical or nursing students/interns and other medical personnel on matters pertaining to pharmacy. Participate in Pharmacy Point of Sale (POS) billing and reimbursement when necessary. Assure the availability (procures cost-effective, quality drugs for timely dispensing), preparation (prepackages and bulk compounds for departmental efficiency), and control of medications (unit-dose repackaging, DEA requirements for controlled substances, removal of outdated drugs from the facility). Perform applicable physical assessments when indicated. Administer vaccines when applicable. Perform all outpatient pharmacist duties if necessary. Perform/complete all other duties and projects as assigned by a pharmacy supervisor. Improvements and Reporting: Plan improvements and direct the delivery of pharmacy services which are conforming to all laws, regulations, standards, and guidelines from applicable organizations. Report, track, and document interventions and contributions to relative’s care made in integrated team. Promote antimicrobial stewardship via education and intervention. Maintain established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs. Coordination of and/or participation in competency assessments of integrated team members or pharmacy staff members. Enter non-formulary drug requests when appropriate with all necessary information including cost considerations and referral to relative benefits coordinator. Perform non-formulary drug reviews on relatives within clinic. Coordinate patient assistance applications with appropriate staff. Initiate prior authorizations with insurance companies if necessary. Prepare drug monographs and cost analyses for presentation to committees, physicians, and other staff. Possess and maintain knowledge of emerging and current evidence-based practice for applicable chronic disease states and serve as a drug information resource for other healthcare providers and team members. Participate in staff education via submission to newsletters, committees, journal clubs, and in-services. Provide formal and informal education and in-services to members of the medical staff, pharmacy, and nursing as requested. Provide training for pharmacy students and technicians. Learn and follow department and facility policy and procedure. Assess complex situations and identify areas for which policies and procedures need to be developed, improved, or revised. Maintain records of medication errors prevented and utilize reporting system to show value of pharmacy services. Perform drug usage evaluations (DUE), quality assurance / performance improvement (QAPI) activities, and drug formulary review evaluations. Serve on OHC Committees, Teams, and workgroups as assigned. Knowledge of federal, state, and local regulations affecting areas of responsibility. Knowledge of requirements of the Drug Enforcement Administration (DEA), Accreditation Association for Ambulatory Health Care (AAAHC), Centers for Medicare-Medicaid Services (CMS), Health Insurance Portability and Accountability Act (HIPAA), Occupational Safety and Health Administration (OSHA), American Society of Health-System Pharmacists (ASHP), the Office of the Inspector General (OIG), and the United States Pharmacopeia (USP). Knowledge of current pharmaceutical practices and principles including pharmaceuticals, disease states, and treatment guidelines. Knowledge of available vaccinations, age requirements, and administration protocols. Knowledge of medications. Knowledge of exposure control including blood borne pathogens and respiratory protection protocols. Skill in establishing priorities and organizing work. Skill in dealing with the public. Skill in oral and written communication. Maintain effective, collaborative interdepartmental and external relations through interpersonal skills. Skill in use of job-related equipment and diagnostic tools. Skill in use of personal computer and a variety of job-related software applications. Supervisory Controls The supervisor provides administrative direction with assignments in terms of broadly defined goals or functions. The employee usually has responsibility for independently planning, designing, and carrying out programs, projects, studies, or other work. Results of the work are considered technically authoritative and are normally accepted without significant change. If the work should be reviewed, the review concerns such matters as fulfillment of program objectives, effect of advice and influence on the overall program, or the contribution to the advancement of technology. Recommendations for new projects and alteration of objectives usually are evaluated for such considerations as availability of funds and other resources, broad program goals, or established priorities. Guidelines and Requirements Guidelines for performing the work are scarce or of limited use. Administrative policies, precedents, laws, and regulations are applicable but are stated in general terms. The employee uses initiative and resourcefulness in deviating from traditional methods or researching trends and patterns to develop new methods, criteria, or proposed new policies. Guidelines include: state and federal laws; regulatory agency requirements; GPTLHB, OHC, and department policies and procedures, including but not limited to ASP Guidelines and clinic policies. These guidelines require judgment, selection, and interpretation in application. This position assists with the development of departmental and health service policies and guidelines. A requirement of this position is to be scheduled according to the needs of a growing and expanding integrated team. This includes the ability to maintain a flexible work schedule, including, evenings, weekends, holidays, and overnight or extended travel as necessary. Complexity/Scope of Work The work typically includes varied duties that require many different and unrelated processes and methods, such as those relating to well established aspects of an administrative or professional field. Decisions regarding what needs to be done include the assessment of unusual circumstances, variations in approach, and incomplete or conflicting data. The continuous changes in healthcare and treatment options contributes to the complexity of the work. The purpose of this position is to deliver the highest quality of pharmaceutical care to OHC relatives. Successful performance helps ensure the overall effective and efficient operation of the pharmacy and OHC resulting in improved relative outcomes. Contacts Contacts are typically with clinical providers, relatives/family members, employees, community resource agencies, various Health Departments, Tribal Health programs, and the general public. The purpose is to influence, motivate, interrogate, or control persons or groups. The employee must be skillful in approaching the individual or group in order to obtain the desired effect, such as gaining compliance with established policies and regulations by persuasion or negotiation or gaining information by establishing rapport. Contacts are primarily for obtaining, furnishing, exchanging, and providing factual medical information. Work Environment/Physical Demands The work is typically performed standing or sitting. The work is typically performed in a clinic setting which requires normal safety precaution for infection control and environmental safety measures. The work environment is usually moderate. While performing the duties of this job, the employee is regularly required to stand and sit, talk, read, write, and hear. The employee is frequently required to reach with hands and arms. The employee is occasionally required to walk and stoop, kneel, crouch; or drive an automobile. The employee must regularly lift and/or move light objects, and occasional lift heavier objects.  Specific vision abilities required by this job include close vision, color vision, and ability to adjust focus.  There is additional risk as this position will work with violent and emotionally disturbed patients, or substance abusers. The work involves moderate risks or discomforts that require special safety precautions, including exposure to contagious diseases. Employees may be required to use protective clothing, such as masks, gowns, coats, boots, goggles, gloves, or shields.
