LICENSED CLINICAL SOCIAL WORKER

DESCRIPTION

The Licensed Clinical Social Worker is responsible for conducting detailed assessments and diagnoses of behavioral health clients, alongside crafting and implementing effective treatment plans to help clients meet their goals. The role requires the ability to evaluate and manage crisis situations with appropriate interventions and actively engage in collaborative meetings that uphold Empower "U", Inc.'s integrated health care philosophy, ensuring comprehensive client care. The candidate will play a crucial role in identifying patients at risk of further psychological or physical decline and contributing to the development of preventative plans. Additionally, this position involves supporting the primary care team by incorporating care management strategies, such as guidelines, disease management, case management, and patient education to enhance chronic disease self-management. Responsibilities also include providing assessments, consultations, and brief interventions for psychological/psychiatric issues, educating patients, families, and staff on care, prevention, and treatment enhancement techniques, and overseeing the behavioral health program to improve patient services. The candidate will deliver therapeutic services to both adults and children, among other assigned duties.

QUALIFICATIONS

• Master’s Degree in social work from an accredited school
• Licensed in the State of Florida
• Two years of supervisory social work experience in a community health care setting, working directly with individuals
• Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with patients, staff and support agencies
• Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the community
• Must possess the ability to make independent decisions when circumstances warrant such action.
