$110,000.00 - $ $127,119.00
Supervisor: Medical Director
POSITION SUMMARY:
Provides medical care and treatment to Brockton Neighborhood Health Center patients in accordance with professional standards and established Brockton Neighborhood Health Center policies and procedures. Provides direct patient care services within area of expertise. Assesses, plans, documents, and evaluates patient needs and treatments. Ensures proper follow up. Maintains practice that is culturally and linguistically sensitive, using interpreters and other assistance as appropriate. Designs a plan of care based on assessment of physical, emotional, and social needs and in collaboration with the patient, and if appropriate, the family. Refers patients, as appropriate, to specialists and other practitioners and facilities. Reviews patient charts and ensures that appropriate treatment plans are developed and followed. Collaboratively works with other caregivers regarding patient education and rehabilitation including promotion of mental and physical health. Maintains collaborative relationship with other physicians, administration, and other clinical staff. Furnishes professional guidance and assistance to colleagues and other staff. Utilizes skills and expertise of other disciplines to ensure comprehensive care for all patients. Cooperates with efforts of the Patient Care Committee to monitor and improve the quality of patient care provided.
Principal Duties and Responsibilities:
1. Conducts comprehensive outpatient care including well child examinations, immunizations and episodic sick visits within the confines of the Center’s objective and policies in collaboration with the pediatrician
2. Initiates follow-up and specialty referrals for those clients needing further work-up or monitoring of a current situation. Initiates and arranges appropriate in-house referrals for medical and non-medical conditions which affect the client’s health.
3. Provides assessment, education and management for comprehensive well child care including health promotion, illness prevention, anticipatory guidance, developmental assessments, enhancement of parenting skills, screening tests and immunizations.
4. Educates patients so that they develop an understanding of the importance of regular well child visits and up-to-date immunizations, and routine adolescent health maintenance.
5. Coordinates the development and review of education classes including normal infant/child growth and development, common childhood illnesses, basic parenting skills and programs addressing the needs of the adolescent population in the area of sexuality, relationships, pregnancy and sexually transmitted disease.
6. Collaborates in the development and revision of departmental policies, procedures, and protocols.
7. Incorporates case management activities into practice and collaborates with multi- disciplinary health care team to provide continuity of care of patients.
8. Maintains harmonious working relationships with medical, administrative and nursing staff, patients and the general public.
9. Participates in discussions with other clinical and administrative staff around the development of goals, objectives and plans and the resolution of problems in the pediatric health service.
MINIMUM SKILLS AND KNOWLEDGE REQUIREMENTS:
Current Massachusetts licensure for RN in expanded role. Certification eligible as Primary Care Practitioner. Federal and State licensure for prescriptive writing privileges.
