Clinical Pharmacist – Stars Program (New York)
Position Description

Overview
The Clinical Pharmacist reports to the Vice President of Quality Improvement and Risk Adjustment with accountability for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE requirements. At CINQCARE, we don’t have patients or customers – we have Family Members.

The Clinical Pharmacist will closely monitor and manage to ensure 4 Star performance the members and their associated Providers in the Stars Program pharmacy related measures. The Clinical Pharmacist will work directly with Members, providers and healthcare professionals within the CINQCARE network as needed. They will be an integral member of the Value-Based Payment (VBP) team, using advanced data and analytics to increase quality metrics around medication adherence and disease management. They will assist with oversight as needed of the IPA/ACO’s Clinical pharmacy vendor.

Job Responsibilities
The Clinical Pharmacist will have the following responsibilities:
  • Monitor on a daily/weekly basis members in the Stars medication adherence measures  and intervene with members, providers and pharmacies as needed to ensure no critical refill dates are missed, 100 day prescriptions have been written and that medications are dispensed timely.  
    • This will include Understanding and being able to explain to Senior Leaders the clinical profile and status relative to receiving the needed care of each non-compliant member in the assigned cohort
    • With prescribing practitioners, the development of individualized member adherence plans
  • Assist in the management of the Clinical Pharmacy vendor to ensure their workflows also lead to a high level of performance on the STARs pharmacy related measures        
  • As needed write draft prescription refills and queue to prescribers
  • As needed participate in medication review and reconciliation
  • As needed provide patient education and provider academic detailing
  • Develop prescribing and dispensing protocols for diseases such as hypertension, diabetes, heart failure, and other chronic conditions reflected in performance measures
  • Develop interventions as needed with local retail pharmacies as well as with payers mail order services and the like.
  • Present evidence-based guidelines, quality improvement, and savings opportunities to the CINQCARE network as appropriate
  • Work to develop early-warning system to identify patients who are noncompliant with their medications
  • Enhance the utilization of Surescripts within MEDENT and other EMR tools
  • Deploy PDSA best practices across the CINQCARE network to improve operational and quality metrics
  • Any other duties as directed

General Duties
The Clinical Pharmacist will have the following duties:
  • Leadership: The Clinical Pharmacist will lead in defining and executing strategies and solutions to create business value in, including building a team to design, develop, and execute those strategies and solutions to deliver desired outcomes. The Clinical Pharmacist will assist in the execution of the CINQCARE Connect clinical model and pharmacy protocols by identifying pharmacy opportunities that impact quality and developing pharmacy-related plans across disease states to improve patient outcomes.
  • Strategy: The Clinical Pharmacist will establish the business strategy and roadmap: (1) improve outcomes for CINQCARE Family Members; (2) enhance the efficacy of other CINQCARE. business divisions; and (3) develop and deliver external market opportunities for CINQCARE products and services. In establishing the business strategy, the Clinical Pharmacist will define and innovate sustainable revenue models to drive profitability of the Company.
  • Collaboration: The Clinical Pharmacist will ensure that capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of CINQCARE.
  • Knowledge: The Clinical Pharmacist will provide subject matter expertise in the solutions, including determining and recommended approaches, solution deployment and performance evaluation.
  • Culture: The Clinical Pharmacist is accountable for creating a productive, collaborative, safe and inclusive work environment for the team and as part of the larger Company.
Qualifications
The Clinical Pharmacist should have the following qualifications:
  • Education: Pharm.D. degree preferred .A Senior Pharmacy Tech with substantial directly related experience would be considered
    • Licensure or Registration in New York State required.
  • Experience: At least 2 years of experience. Ideal candidates will have relevant experience in the healthcare industry.
  • Entrepreneurial: CINQCARE seeks to fix gaps that have persisted for generations in the delivery of care to Black and Brown populations. This position is accountable for ensuring CINQCARE is positioned to innovatively deliver on its promise.
  • Communication: Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner to CINQCARE’s team, investors, partners, and other stakeholders. Proficiency in all Microsoft Office applications.
  • Relationships: Ability to build and effectively manage relationships with business leaders and external constituents.
  • Culture: Good judgement, impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced, demanding, and entrepreneurial Company.

Location: New York State. May require occasional but infrequent travel into the Buffalo or New York City Office locations.

Compensation: $135K to $170k
