Licensed Vocational Nurse/ LPN

.Job Summary:
The LVN/LPN will provide skilled nursing care to patients in a home health setting under the direction of the Registered Nurse. The LVN/LPN will collaborate with the interdisciplinary team to develop and implement care plans and provide quality and compassionate care to patients and their families. This is a full-time individual contributor role located at the Interim HealthCare franchise in San Rafael, California.

Compensation & Benefits:
The LVN/LPN position at Interim HealthCare offers a competitive salary of $35 to $42 per year, paid weekly. Additionally, we offer many comprehensive benefits package.

Responsibilities:
- Provide skilled nursing care to patients in their homes according to the care plan developed by the Registered Nurse.
- Administer medications and treatments as prescribed by the physician.
- Monitor and document patient's progress and communicate changes to the interdisciplinary team.
- Teach and educate patients and their families about their care, disease management, and medication.
- Collaborate with other healthcare professionals to ensure the holistic needs of the patient are met.
- Maintain accurate and up-to-date patient records.
- Adhere to all company policies, procedures, and regulatory requirements.

Requirements:
- Current LVN/LPN license in the state of California.
- Minimum of one year of nursing experience required.
- Excellent written and verbal communication skills.
- Ability to work independently and as a team player.
- Current CPR certification.
- Ability to lift and transfer patients safely.
