LPN Intake Nurse

Horizon Healthcare Staffing, a NJ leader in School Nursing Services to public and private schools, is seeking a Licensed Practical Nurse (LPN) to add to our internal staff (Monday through Friday normal business hours) in our Manalapan, NJ office.

Job Type: Temporary

Assignment: Monday – Friday (flexible) 37.5 Hours

Starting ASAP- September

RESPONSIBILITIES

The Intake Nurse will be responsible for:

• Processing all assigned incoming intake referrals.
• Ensuring all referrals are clinically complete, efficient, and accurate.
• Receiving job orders/contact forms for correspondence tracking.
• Contacting Case Manager assigned to client/student of the job order for questions or clarification.
• Contacting school district listed on request form to confirm information and request to conduct an intake (assessment).
• Calling parent/guardian to complete intake assessment.
• Requesting MD orders.
• Sending parent/guardian HIPPA consents and parent packets with Horizon’s information.
• Following up with parent/guardian if information needed has not yet been received to proceed with the case.
• Following up with various school districts.
• Ensuring open communication
• Addressing clinical issues as needed.

QUALIFICATIONS

Qualifications:

• NJ Licensed Practical Nurse (LPN) License.
• Minimum year of clinical experience.
• Excellent phone etiquette.
• Outstanding verbal and written communication skills.
• Exceptional computer skills including Microsoft Office.
