Description:

A. POSITION SUMMARY:

The Nursing Assistant assists the resident in performing activities of daily living, provides a clean and safe living environment, gathers data on the resident’s physical and emotional state, and reports observations to the nurse in charge.

  • Assists the resident with activities of daily living:

A. Bathes or assists resident to bathe, provides evening care, and incontinence care.

B. Grooming; including care of hair and fingernails.

C. Assists in dressing and undressing.

D. Assists with meal tray, feeds resident as needed, and provides between meal nourishments and fluids as assigned.

E. Toilets resident.

F. Positions resident for comfort, using principles of body mechanics and repositions when necessary.

G. Provides or assists with oral hygiene.

H. Answers signal lights in a prompt manner.

  • Recognizes resident’s strengths and uses rehabilitative nursing techniques to restore or maintain the resident's independence:

A. Range of motion exercises.

B. Supervises and/or assists with ambulation, transfer, etc.

C. Assists with bowel and bladder training.

D. Participates in reality orientation.

  • Assists in meeting the resident's socio-emotional needs:

A. Explains procedures and communicates with resident.

B. Shows support through listening.

C. Encourages social and/or therapeutic activities.

D. Supports the dying resident.

E. Provides for residents’ privacy when appropriate.

F. Ensures the resident's rights are maintained.

G. Lights cigarettes and pipes and attends residents when they smoke (if applicable).


  • Assists in provision of a safe and clean environment:

A. Properly stores and uses equipment.

B. Reports faulty equipment and accidents to Charge Nurse.

C. Dresses resident appropriately for activities, appointments, etc.

D. Makes and remakes beds as assigned.

E. Cleans beds, bedside units, and diaper hampers, and all other areas designated by Charge Nurse.

F. Puts away clean linen and supplies.

  • Assists in preventing the spread of infection:

A. Uses proper hand washing techniques.

B. Uses proper disposition of soiled linen.

C. Cares for residents in isolation.

  • Reports observations to Charge Nurse concerning the resident’s condition or activities:

A. Participates as a team member in Nursing Care Conference.

B. Records observations on nursing worksheets as assigned.

  • Performs postmortem care:

A. Attends Staff Development programs related to job responsibilities.

B. Transports residents from one nursing unit to another.

C. Accompanies residents to the patio.

D. Performs other related duties as assigned and may rotate among various units.


B. QUALIFICATIONS:

  • High school Diploma required.
  • Ability to read, write, and follow directions in English for business necessity.
  • Must be able to communicate effectively with residents and staff.
  • Interest in the geriatric patient necessary.
  • Previous healthcare facility experience preferred.
Requirements:

STANDARD OF PERFORMANCE

A: Clinical

Activity:

  1. Performs routine patient care in accordance with Company and Center policies and nursing procedures
  2. Employs preventative and maintenance measures.
  3. Ensures that adequate nutritional and fluid intake is provided. Assists patients as needed.
  4. Assist in patient admission, discharge, and transfer as instructed.
  5. Complies with safety policies and practices (i.e., gait belts).
  6. Uses equipment correctly and safely during the performance of direct patient care.
  7. Follows hand washing techniques and other infection control guidelines during the performance of patient care
  8. Makes frequent observations for indications of change of patient condition during performance of patient care.

Measured By:

  1. Direct observation by Patient Care Supervisor.
  2. Direct observation by Patient Care Supervisor.
  3. Observation of tray by Patient Care Supervisor and documentation of intake.
  4. Direct observation by Patient Care Supervisor and review of documentation on admission, discharge, and transfer.
  5. Lack of employee or patient related accidents or injuries, review Safety Committee Reports, and direct observation by Patient Care Supervisor.
  6. Direct observation by Patient Care Supervisor and review of Incident Reports
  7. Direct observation by Patient Care Supervisor and review of Incident Reports
  8. Direct observation by Patient Care Supervisor and review of resident Physical Assessment made by the Patient Care Supervisor.

B. ADMINISTRATIVE

Activity:

  1. Reports any change of patient condition or other concerns to Patient Care Supervisor in a prompt manner.
  2. Prioritizes patient care activities based on current care needs.
  3. Follows charting guidelines to document care in Patient Health Records.
  4. Participates in all assigned meetings.
  5. Participates in all training, education, and/or in-service as required.


Measured By:

  1. Direct observation by Patient Care Supervisor and verbal feedback from patients.
  2. Time and Attendance Records, direct observation by Patient Care Supervisor, and verbal feedback from patients
  3. Direct observation by Patient Care Supervisor, review of documentation by Patient Care Supervisor, and Health Records audits.
  4. Review of minutes and Attendance Records by nursing management.
  5. Review of in-service records by Director of Staff Development

C. CONSUMER SERVICE:

Activity:

  1. Reports for duty in approved uniform and maintains high standards of personal hygiene
  2. Refrains from rude and discourteous behavior towards consumers and co-workers. Avoids the use of profanity.
  3. Limits eating and/or smoking to designated areas.
  4. Reports to duty promptly as scheduled. Takes breaks only during assigned times and in designated places.
  5. Speaks, writes, and understands English for business necessity.
  6. Follows consumer service standards for:
  • Making daily contact with patients;
  • Greeting visitors;
  • Making initial contact with newly admitted admitted residents;
  • Giving information to patients and visitors;
  • Getting information from patients and visitors;
  • Referring patients and visitors; and
  • Handling complaints

7. Utilizes the telephone in accordance with consumer service standards when:

  • Answering phones;
  • Placing a call on hold;
  • Transferring a call; and
  • Taking a message

8. Protects patient rights through monitoring general patient condition to identify, correct, and/or report infringement of rights.

  1. Visual inspections by Patient Care Supervisor or other management staff.
  2. Direct observation by Patient Care Supervisor or other management staff (and) absence of complaints.
  3. Direct observation by Patient Care Supervisor or other management staff (and) absence of complaints.
  4. Review of time and attendance records, direct observation by Patient Care Supervisor or other management staff (and) absence of complaints.
  5. Direct observation by Patient Care Supervisor or other management staff (and) absence of complaints.
  6. A combination of absence of complaints (and) periodic audit of skills by Patient Care Supervisor or other management staff.
