Overview:

We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our team as a Behavioral Health Case Manager. In this role, you will provide comprehensive case management services to individuals with behavioral health needs, helping them access and navigate appropriate resources and services to support their well-being.

Responsibilities:

  • Conduct thorough assessments of clients' behavioral health needs, including mental health, substance abuse, and social determinants of health.
  • Develop and implement individualized care plans in collaboration with clients, their families, and interdisciplinary team members.
  • Coordinate and monitor the delivery of services and interventions to ensure alignment with clients' treatment goals and objectives.
  • Provide crisis intervention and support to clients experiencing acute behavioral health crises.
  • Advocate for clients' rights and access to quality behavioral health care services within the community.
  • Maintain accurate and up-to-date documentation of client interactions, assessments, care plans, and progress notes in accordance with regulatory standards.
  • Collaborate with community partners, including healthcare providers, social service agencies, and other stakeholders, to facilitate seamless transitions of care and continuity of services.
  • Participate in multidisciplinary team meetings, case conferences, and trainings to enhance clinical knowledge and skills.

Qualifications:

  • Master's degree in Social Work from an accredited institution.
  • Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of [State].
  • Minimum of 3years of experience providing case management services in a behavioral health setting.
  • Strong knowledge of mental health and substance abuse disorders, treatment modalities, and evidence-based practices.
  • Excellent assessment, crisis intervention, and communication skills.
  • Ability to work collaboratively within a multidisciplinary team and independently manage a caseload.
  • Proficiency in electronic health record (EHR) systems and Microsoft Office Suite.
