The HLS Health Education Specialist contributes directly to the overall coordination and implementation of the HLS program components, under the supervision of the Prevention and Wellness Manager. This includes program planning, implementation, one-on-one client services and facilitating linkage to HIV testing and healthcare services (for both HIV positive and negative clients). In particular, the HLS Health Education Specialist will coordinate and facilitate wellness programs for HIV negative clients and will provide enrollment in public health insurance or access programs and linkages to MNHC or another medical home.

***Starting hourly range is $19.00 to $23.00 with benefits***

Essential Functions & Responsibilities

The HLS Health Education Specialist has the following responsibilities and duties:

Direct Services - 60%

  • Conduct initial screening and Behavioral Risk Assessments of new participants.
  • Co-Facilitate Initial Risk Reduction Group and Intensive Drivers Workshops, in accordance with established program protocols.
  • Coordinate timely implementation of 4 month follow-up of Behavioral Risk Assessments for participants of the Initial Group. Administer follow-up BRA.
  • Plan content/topics for the HLS Follow-up groups in accordance with participant's needs and program goals. Co-Facilitate follow-up groups and develop written process documentation for each group.
  • Provide screening and enrollment in Healthy San Francisco (HSF) or other public health insurance/access programs for eligible HIV- clients. Assist the client in registering at MNHC or other medical home and follow-up to ensure the client is engaged in primary care.
  • Provide HIV Testing/Counseling Services as required by group participants. Support Treatment Outreach Worker and Treatment Link Specialist in the conduction of outreach and other tasks to increase HIV testing rates among at-risk populations.
  • Work with clients at increased risk of HIV to assess eligibility for and enroll clients in PrEP and PEP.
  • Assist with client charts, update records and input patient-level data during PrEP Clinic.

Program Coordination & Development - 40%
  • With the assistance of the Prevention and Wellness Manager, develop and coordinate a yearly Outreach Plan for the program.
  • Assist with the evaluation of Intensive Drivers Reduction Workshops and Follow-up groups on an Annual basis and coordinate timely implementation of data to Evaluation Consultant. Work closely with consultant in Outcome Evaluation of program.
  • Maintain statistical data; develop monthly reports for HIV Prevention and Wellness Manager.
  • Actively participate in various weekly HIV Services staff meetings, in-services, and trainings as well as required, and MNHC meetings (All-Staff, etc.)
  • Along with prevention and wellness staff, co-produce outreach and recruitment events to bring visibility to workshops, groups and PrEP/PEP program.
  • Additionally, the HIV Community Health Educator represents Mission Neighborhood Health Center in community planning groups, advisory boards, committees and conferences.
  • Other duties as assigned by supervisor.

Qualifications & Requirements

The HLS Health Education Specialist should have the following skills, education, and experience:
  • Bachelor's degree preferred. AA and experience considered.
  • Fluency in Spanish/English both oral and written required.
  • Minimum 1 year experience in HIV Prevention or Public Health.
  • Demonstrated ability to work independently and show effective judgment in dealing with a diverse population with varied needs.
  • Sensitivity to HIV/AIDS and Gay, Lesbian, Bisexual, and Transgender issues.
