As a member of the team, you will provide short-term services to assist patients and families in coping with personal, financial, psychosocial, and environmental problems associated with the patient's medical and/or psychiatric diagnosis. What you'll need:

• Washington State Licensed Clinical Independent Social Worker,
• Two (2) years recent LCISW experience in health care setting
• Minimum two years Social Worker experience in healthcare setting, clinical social work in similar setting.
• Experience in working with specific patient populations that may include but are not limited to: children, frail elderly, vulnerable adults, and patients impacted by mental illness
• Previous experience in AIMS collaborative care primary care clinic is a plus.
• May require valid Washington state driver's license.

Schedule: 1.0 FTE; 40 hours/week; Day Shift;
