Job Summary
As a Medical Social Worker, you will provide medical social services to patients in their place of residence in accordance with the Plan of Care. You will assist the physician and other team members in understanding the significant social and emotional factors to the health problems.
The work schedule and shift expectations are based on assigned caseload.
Medical Social Worker will be required to drive daily for meetings and/or visits to residential homes.
Coverage Area: Arlington Heights & surrounding (will need to cover entire territory. No further north than Crystal Lake Route 167, not east of I-294).
Requirements
  • Illinois Licensed Clinical Social Worker (LCSW) or LicensedSocial Worker (LSW)
  • Master's Degree in Social Work or related field
  • 2 years of post-grad social work experience in a healthcare setting
  • Home Health experience preferably with the same region
  • Current driver's license
  • Ability to drive to/from patient locations
  • The skills needed to self-manage your time and schedule
  • Demonstrated experience with tablets, mobile phones and EMR software
