Your Role as a Home Health Medical Social Worker Intern (MSW)

As a Home Health Medical Social Worker Intern (MSW), you will provide psychosocial support and social services to patients receiving care in Assisted Living Facilities. You will play an important role in identifying social, emotional, financial, and environmental factors that may impact patient outcomes and overall well-being.


Responsibilities Include:

• Assist physicians and interdisciplinary team members in understanding social and emotional factors impacting health conditions
• Develop and coordinate care plans addressing psychosocial needs
• Connect patients and families with community resources and support services
• Provide counseling, education, and emotional support to patients and caregivers
• Assist with discharge planning and care transitions
• Collaborate with physicians and interdisciplinary care teams to improve patient outcomes
• Maintain timely and accurate documentation in accordance with Medicare guidelines and agency policies
• Complete documentation efficiently using eClinicalWorks(eCW) EMR

Qualifications

• Working toward a Master’s or Doctoral degree from a school of social work accredited by the Council on Social Work Education
• Minimum one year of social work experience in a healthcare setting required
• Home health experience preferred
• Strong assessment, communication, and care planning skills
• Ability to work independently while collaborating with an interdisciplinary team
• Valid driver’s license, reliable transportation, and current auto insurance
