REGISTERED PHYSICAL THERAPIST
Reports to: DON/Clinical Supervisor
Job Summary:
Professional member of patient’s treatment team who evaluates, assesses, and delivers services according to a written plan of treatment established by a physician.
Provides supervision for all services rendered by Physical Therapy Assistants. All performance evaluations are conducted by peers. Participates in coordination of care for all assigned patients.
Standard responsibilities of the Registered Physical Therapist
- Ongoing interdisciplinary assessment of the patient
- Development and evaluation of the POC in partnership with the patient, representative and caregiver
- Able to perform and complete OASIS assessments
- Providing services that are ordered by the physician as indicated in the POC
- Patient, caregiver and family counseling
- Patient and caregiver education
- Preparing clinical notes
- Communicating with the physician who is responsible for the POC and with other health care practitioners related to the current plan of care
- Participation in Agency QAPI
- Participation in Agency sponsored in-service training
Other duties and responsibilities:
1. At least every 30 days functionally reassess patient for on-going therapy needs.
2. Supervise PTA every 30 days in carrying out PT program. Coordinates methods to assure patient needs are met.
3. Establish/instruct in home exercise program (HEP); document instructions on visit notes.
4. Observe and report to physician pertinent changes in the patient’s level of functioning. Communicate changes to other team members involved in the patient’s care.
5. Advises and consults with family and other agency personnel.
6. Accept only those assignments for which qualified.
7. Participates in in-service programs.
8. Maintain all patient/Agency information in strictest confidence.
9. Respect patient rights, cultural beliefs.
Qualifications:
1. Graduated after successful completion of Physical Therapy education program approved by one of the following:
a. The Commission on Accreditation in Physical Therapy Education (CAPTE)
b. Successor organization of CAPTE
2. Licensed/registered in the State of Florida as a Physical Therapist
Risk category II: Routine tasks do not involve exposure to blood, fluids of tissue, but employment may require performing unplanned Category I tasks which would include exposure to blood, mucous membranes, tissues. In those cases, appropriate protective measures are required.
Physical Requirements:
Ability to read, write, hear and verbally communicate in English to the degree required to perform job duties.
Ability to stand, walk, sit, push, pull, kneel, crouch.
Stoop ability to lift/carry, push/pull more than 100#.
Ability to withstand wet/humid, non weather related environments.
