Job Summary:
A Licensed Practical Nurse is a professional who plans and manages patient care according to each individual’s needs. They interview patients and record their medical history and physical conditions like blood pressure or temperature while obtaining vital signs.
This position reports to the Director of Health Services.
Duties/Responsibilities:
· Provide supervision of participants in all areas of the agency or community where participants are involved.
· Provide all medical or healthcare services that are integral to the participant’s activities of daily living (ADL).
· Provide supervision and monitoring of the participants to assure that health care needs are being met including supervision of medication.
· Provide first aid and making arrangements for medical care with a physician or hospital in case of accidents or medical emergencies.
· Regular attendance at program meetings to discuss plans and developments affecting program area.
· Must complete required training for administration of participant medications (when applicable)
· Must be able to assist participants in need of assistance with their personal care and other daily living activities of daily living (ADL).
· Maintain on a current basis all documentation required by the applicable Medicaid waiver including: the monthly summary notes and medication administration records.
· Monitoring of the supports provided to all waiver recipients.
· Performs other duties as assigned.
Required Skills/Abilities:
· Excellent verbal and written communication skills.
· Excellent organizational skills and attention to detail.
· Proficient with Microsoft Office Suite or related software.
Education and Experience:
· Must have current and valid LPN license with a minimum of one (1) year experience.
· One-year experience in the field of developmental disabilities
· Must be able to acquire and maintain CPR and First Aid Certification.
Physical Requirements:
· Alternating periods of sitting, standing and walking for up to 8 hour shifts.
· Must be able to lift up to 50 pounds at times.
