Job Description

Position Summary

The BHC Clinical Manager directly manages master’s-level Behavioral Health Consultants (BHCs) providing behavior change services within an integrated primary care setting. This position provides individual and group clinical supervision and ensures that staff are trained on, and use, trauma-informed, evidence-based models of care. Develops, maintains, and reports on program evaluation metrics and ensures program fidelity. Provides behavioral health consultation services within the primary care clinic.

Duties and Responsibilities

  1. Provide supervision, including monitoring and evaluation, to team of BHCs.
  2. Champion Primary Care Behavioral Health (PCBH) model of care.
  3. Orient and train new staff on PCBH model through various means.
  4. Oversee and ensure staff productivity and clinical direction of PCBH work.
  5. Work with the Quality department to measure, monitor, and ensure strong clinical and related program outcomes.
  6. Work in coordination with administrative staff to assure accurate insurance and billing by staff.
  1. Knowledge of psychotropic and other relevant medications used in the primary care setting.
  2. Maintain 0.60 FTE of behavioral health consultation patient care in the primary care clinic.
  1. Provide assessments and interventions to patients in the primary care clinic for any health problem influenced by behavior.
  2. Conduct risk assessments, as indicated.
  3. Ensure availability for same-day and scheduled behavioral change visits with patients, and attempt to see all referred patients on the same day (if in clinic when the patient is referred).
  4. Maintain a visible presence to the PCPs during clinic operating hours.
  5. Provide availability for “curbside” consultation (a brief interaction between the BHC and a PCP) by being in the clinic or available by phone or athenaText.
  6. Perform follow-up visits within a consultative structure.
  7. Regularly utilize strategies to grow the breadth and productivity of the PCBH service.
  8. Provide psycho-education for patients during individual and group visits.
  9. Assist in the development of clinical pathway programs, group medical appointments, classes, and behavior-focused practice protocols.
  10. Other duties and responsibilities, as assigned.


Required Skills or Abilities

  1. Proficient in Microsoft Office Suite and ability to work in an electronic health record.
  2. Proven ability to communicate and foster effective working relationships with both providers and patients.
  3. Ability to work with a diverse patient and staff population.
  4. Present a positive, professional image to persons who have diverse interests and ideas.
  5. Active BLS Certification.
  6. Bilingual in Spanish or Polish preferred

Qualifications, Knowledge, Experience, or Licensure

  1. Master’s degree in social work/psychology from an accredited institution.
  2. LCSW, LCPC or PsyD license in state of Illinois.
  3. 5-7 years of post-graduate clinical work within a behavioral health setting.
  4. 1-3 years of supervisory experience within the behavioral health environment.
  5. Understanding of the primary care clinic setting and behavioral health integration.

Physical Demands

  1. Must be able to remain in a stationary position 50% of the time.
  2. Must be able to move around the clinic site 50% of the time.
  3. Constantly operates a computer, computer printer, copy machine, and telephone.
  4. Occasionally positions self to maintain exertion of physical strength to move objects of 10 pounds from one level to another.
  5. Must be able to transport from one site to another.
  6. Must be able to cover other shifts as necessary; one evening may be required.
