Job Description

General Summary

This is a Breast oncology Nurse Navigator position.  This role is designed for the nurse who has achieved expertise and leadership in the clinical and professional role.  Leads and evaluates evidence based practice, actively participates and/or leads organizational initiatives, mentors and coaches nurses and other disciplines, exemplifies the role of the professional nurse and demonstrates active involvement in clinical and professional nursing beyond the walls of the unit and/or organization. 

Principal Responsibilities and Tasks

The following statements are intended to describe the general nature and level of work being performed by people assigned to this classification.  They are not to be construed as an exhaustive list of all job duties performed by personnel so classified.

  • Initiates contact with all new breast cancer patients requiring multimodality therapy. Works with the UMGCCC referral center and MDs to direct new patients to the appropriate provider and clinic.
  • Coordinates with clinical care team to identify and navigate patients through multi-D and high risk breast clinic(s).
  • Incorporate activities to ensure delivery of quality patient care, the attainment of departmental objectives and promotion of the positive patient experience.
  • Collaborates closely with MDs, Breast Leadership team and nursing staff such as surgical, radiation and medical oncology nurse coordinators to assure smooth transfer of care from the multi-D setting to other oncology services for appropriate therapies to be initiated.
  • Leverages local and national education (NCI, ASCO, NCCN, and AONN) materials to assists patients in understanding their diagnosis, treatment options and the resources available, including educating patients about clinical research studies.
  • Connects patients to the following service areas (not limited to): financial counseling, American Cancer Society navigator, chaplaincy, social services, dietary, ancillary departments, and support groups as indicated.
  • Follows patients throughout the care continuum to ensure progression of the plan of care involved.
  • Partners with Breast Leadership Team and EPIC partners to collect, analyze and support NAPBC accreditation standards
  • Works in partnership with the administrative director and breast physician leadership to define, evaluate and implement quality measures and strategic goals for the program
  • Engages with survivorship program leaders to ensure all patients with a breast cancer diagnosis receive a Survivorship Care Plan by identifying and monitoring activity via tumor registry and journey forward care plan builder
  • Quarterly attendance at Baltimore city cancer program screening day
  • Attendance and participation of at least one community screening or prevention event yearly
  • Quarterly social media activity with the assistance of marketing discussing health disparity, maintenance or prevention topics focused on breast cancer
Qualifications

Education & Experience

  1. Licensure as a Registered Nurse in the state of Maryland, or eligible to practice due to Compact state agreements outlined through the MD Board of Nursing, is required.​​​​​​
  2. Bachelor’s degree required. BSN required for hires after July 1, 2008 
    • Nurses on UMMC payroll by 7/1/08 must have a Bachelor’s degree or active enrollment with graduation within 5 years of promotion. Bachelor’s degree can be in any field through June 2012.  External candidates must have BSN.
  3. Current CPR certification required.
  4. Two years professional nursing experience required, ambulatory/outpatient experience preferred. 

  5. Recent applicable clinical experience in a comparable setting.

  6. Maintenance of 15 contact hours of education every year or 30 contact hours of education every two years.

  7. Member of a professional organization. 

  8. Oncology Certfified Nurse (OCN) certified preferred

Knowledge, Skills and Abilities

  1. Demonstrated ability to lead others, such as effectiveness as charge nurse, preceptor, or clinical   resource. 
  2. Demonstrated effectiveness as a multidisciplinary team member.
  3. Ability to learn and use computer systems in order to manage patient information.
  4. Ability to demonstrate knowledge and skills necessary to provide care appropriate to the patient population(s) served.  Ability to demonstrate knowledge of the principles of growth and development over the life span and ability to assess data reflective of the patient's requirements relative to his or her population-specific and age specific needs.   
  5. Demonstrated/documented highly effective interpersonal and verbal and written communication skills.

Patient Safety

Ensures patient safety in the performance of job functions and through participation in hospital, department or unit patient safety initiatives.

  1. Takes action to correct observed risks to patient safety.
  2. Reports adverse events and near misses to appropriate management authority.
  3. Identifies possible risks in processes, procedures, devices and communicates the same to those in charge.
