Adult & Child Health is an integrated healthcare provider in Central Indiana. Providing high-quality, affordable care including primary care, behavioral health services, substance abuse treatment, mobile crisis response, community-based services, homeless outreach, school-based treatment, and more.

About this role: The Housing Stability and Engagement Coordinator will provide on-site services at Hanna commons that will include case management, mental and behavioral health treatment, benefits assistance, and referrals to primary care through its offsite FQHC. The Housing Stability and Engagement Coordinator will also manage referrals to other services, both on- and off-site.

The Location and hours: A&C is based in Central Indiana. For this position, we service the following county: Johnson

Hours are M-F 9-5pm with flexible scheduling available.

Responsibilities include but not limited to:

  • Providing collaborative planning, referral and monitoring of treatment and support services.
  • Connecting persons with resources to address immediate needs including but not limited to: primary care, home healthcare, mental/behavioral healthcare treatment, entitlements, and supportive employment resources.
  • Engaging persons in transition to housing.
  • Collaborating and coordinating with community partners, referrals sources, providers and advocating on behalf of persons being served in the project.
  • Completing required documentation and data entry in a clear, accurate, and timely fashion.
Requirements:

  • Strong working knowledge of community resources
  • Bachelor’s degree in Social Work, Psychology or related field, required
  • Experience working with individuals experiencing homelessness
  • Available to work weekdays and some evenings
  • Strong organizational, communication, and writing skills
  • Must hold a valid driver’s license with acceptable driving record and provide own, insured, transportation
