Job Title: Certified Health Worker (CHW)
Salary: $25-$25 per hour DOE
Location: Wellness Center
Openings: 1) Position Available
Position Purpose: Under the supervision of the Behavioral Health Administrative Manager, the Community Health Worker (CHW) is responsible for helping patients and their families navigate and access community services and adopt healthy behaviors. The CHW supports providers and care managers through an integrated approach to case management and community outreach.
Core duties and responsibilities include but are not limited to:
- Advocate for individuals and families to receive access to necessary health and community resources
- Serve as a liaison between patient and care managers
- Conduct outreach and enrollment calls to enroll patients and community members in Enhanced Care Management (ECM) services
- Attend health fairs, events, and other activities as assigned
- Develop relationships with outside organizations
- Provide referrals to the community as appropriate
- Help patients connect with transportation services and provide ongoing appointment reminders
- Work collaboratively with medical providers, social service team members, and other FTOC staff
- Responsible for providing consistent communication to the care managers regarding patient updates
- Record patient care interaction in the EMR within 48 hours
- Manage assigned caseload of patients
- Maintain HIPAA compliance at all times
- Attend regular staff meetings, trainings, and other meetings as required
- Perform other duties and projects as assigned by the Behavioral Health Administrative Manager

Education and Training:
- Bachelor’s degree in social work, psychology, counseling, sociology, or behavioral science
- Bilingual: Spanish Required
Qualifications, Skills, and Experience:
- Minimum 3 years working for a community based organization or community health center
- Must excel in multitasking within a high-paced environment
- Strong computer skills, acute attention to detail, confident and professional communication.
- Knowledge and experience in working with diverse communities
- Resourceful community liaison, experience navigating through complex health systems and community services
