Job Description

Compass Healthcare Consulting & Placement is conducting a search for an experienced RN Clinical Manager for our Client, a Certified Home Health Care Agency with an office located in New Hyde Park, NY. This is a Direct Hire, Full Time position. Position is Hybrid Remote, working up to 3 days in office, and up to 2 days remote, from home. Position also requires occasional Field Visit for 1:1 with Field RN for Education/Training.

Qualified candidates will have prior Certified Home Healthcare Agency experience as an RN, Registered Nurse, RN Case Manager, Coordinator of Care or Patient Service Manager and have the ability to oversee Clinical Field Staff with CHHA knowledge, leadership and experience in OASIS Documentation, Coordination of Care and Certified Home Healthcare Agency experience is required. Position is in the office with occasional need to go in the field to conduct joint visit with Field Staff.

JOB SUMMARY

The Clinical Manager is responsible for ensuring the provision of quality and compliant care to patients, manages a caseload and oversees Field Staff.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

  1. Ensures that patient needs are continually assessed.

  2. Ensures the development, implementation and update of the individualized plan of care.

  3. Oversights and participates in coordination of patient care. This includes facilitating, participating in, and overseeing case conferencing on all patients.

  4. Ensures compliance with state, federal and CHHA regulations and standards at all times.

  5. Provides clinical support to field staff.

  6. Ensures field staff are meeting productivity standards set forth by the Organization.

  7. Works collaboratively with the Administrator/DPS and other agency leadership (including but not limited to Education and Quality Departments) to identify needs for ongoing education and training of staff.

  8. Instructs and guides clinicians to promote more effective performance and delivery of quality home health services.

  9. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities and in developing plan of care.

  10. Conducts ongoing and quarterly record reviews. Works directly with staff to facilitate process improvement, including the implementation of Performance Improvement Plans where deficiencies are identified. Communicates findings and recommendations to appropriate Organization personnel.

  11. Assists in the screening and interviewing process of new Organization personnel and makes recommendations for employment of individuals. Assists in the orientation of new Organization personnel.

  12. Assists in the formulation, revision, implementation and evaluation of policies and procedures, as well as strategic goals and objectives.

  13. Ensures that the Organization’s standards for customer service and service excellence are met and maintained by field staff. This includes the oversight of Organization Patient Satisfaction and the receipt and subsequent investigation and closure of any received complaints.

  14. Performs other duties and activities as delegated.

QUALIFICATIONS

  • Registered Nurse with current NYS license. 

  • Bachelor’s Degree or higher preferred.

  • One (1) to three (3) years of recent certified home health experience.

  • Strong understanding of home health regulations and applicable standards, including OASIS documentation

  • Maintains knowledge of current clinical best practices

  • Demonstrated leadership skills; formal leadership/supervisory experience preferred.

  • Demonstrates excellent observation, verbal and written communication skills.

  • Ability to travel in the Field on Occasion

  • Ability to work both in office and remote from home
